Outline of Research at the Start

現在、脊髄くも膜下麻酔には局所麻酔薬が用いられているが、血圧低下が起こりやすい、作用時間が短い、手術後の運動障害が残るなどの欠点も多い。申請者は局所麻酔薬の代わりにメサドンというオピオイドを用いると上記の欠点を起こすことなく感覚のみを遮断できる可能性があることを発見した。本研究はラットの脊髄スライスからのCaイメージング法やパッチクランプ法を用いて、このメサドンの作用機序を詳細に解析し、メサドンを用いた脊髄くも膜下麻酔が可能であるか検討する。また、メサドンの構造のうち、どの部分がこの作用の主体となっているか解析し、さらに選択的に作用する薬剤を開発する。

Outline of Final Research Achievements

The effects of opioids on dorsal horn cells were examined using rat spinal cord slices. Morphine reached the upper limit of its inhibitory effect on dorsal horn cell excitation at a concentration of 1 microM, and its inhibitory effect was not enhanced at higher concentrations, while methadone completely abolished dorsal horn cell excitation at a concentration of 100 microM. This effect was also observed in the presence of naloxone. Next, patch clamp recordings were made from lamina II neurons to examine the effect of methadone on excitatory postsynaptic currents evoked by dorsal root electrical stimulation. Dorsal root stimulation-induced postsynaptic currents were completely abolished at 300 micromolar methadone, even in the presence of naloxone. It was suggested that methadone acts on primary afferent fiber endings to inhibit glutamate release.

Academic Significance and Societal Importance of the Research Achievements

メサドンというオピオイドを脊髄後角細胞に投与すると、局所麻酔薬のように後根の神経伝導を遮断することなく、脊髄後角細胞の反応を遮断することができた。これまで脊髄くも膜下麻酔は局所麻酔薬を脊髄くも膜下腔に投与することによって行われていたが、メサドンというオピオイドをくも膜下腔に投与することによっても同様の麻酔状態にできることが示唆された。メサドンは交感神経を遮断しないため血圧低下が起こりにくいことが想定され、長時間作用性という性質により、術後も長時間の無痛状態を作れる可能性があることを示した。神経障害がないという安全性が証明されれば、より安全で快適な脊髄くも膜下麻酔が可能かもしれない。
