| Outline of Research at the Start |
近年、世界疾病負担研究(GBD study)において歯科疾患は世界で最も有病率が高い疾患として報告されており、2021年5月には第74回WHO世界保健総会にて口腔保健の決議がなされた。そこで国際的に注目を集めるが、研究が少ない歯科医療に焦点を絞り、複数の国の疫学データの国際比較研究を実施する。そしてUHCの観点から「公的なカバーの手厚さが医療受診格差を減らしている実態」について検証を行う。豊富な国際比較分析により、世界中で活用されるエビデンスの形成を目指す。

| Outline of Final Research Achievements |
Given the background that Japanese individuals, due to their universal health insurance system, are more likely to visit dentists internationally (Aida et al., 2021), we conducted international and domestic research. A study comparing Singapore and Japan demonstrated that the inequalities in tooth loss among the older population were larger in Singapore than in Japan (Kiuchi et al., 2023). A life-course study suggested that economic conditions in adolescence could influence oral health in older age through poor health behaviors (Yamamoto-Kuramoto et al., 2023). Another study suggested that assets might have a greater influence on dental care utilization among older adults than income (Morohoshi et al., 2025).

| Academic Significance and Societal Importance of the Research Achievements |
健康格差は日本でも国際的にも重要な研究テーマである。ユニバーサルヘルスカバレッジ（UHC）は健康格差の文脈から注目されている概念で、国際学術誌においても近年よく取り上げられている。歯科医療はUHCの中に含まれていない国が多く、WHOや国際歯科学会（IADR）は歯科医療をUHCに含める必要性を訴えている。日本は例外的に幅広い歯科医療が国民皆保険に含められており、非常に歯科医療にアクセスしやすい国である。この日本との国際比較研究や日本国内の研究により、歯科疾患の健康格差がUHCにより縮小する可能性や、日本でも存在する格差に対する知見が得られ、国内外の公衆衛生的な社会的意義があったと考えている。
