| Outline of Research at the Start |
本研究は前向き多施設共同観察研究である。FFR＞0.80であった中等度の冠動脈病変（狭窄度30-80%）に対してNIRSを行った患者を登録する。血管の屈曲によりNIRSが行えない場合、血管径＜2mm、インフォームドコンセントが得られない場合は除外する。MaxLCBI4mm (>400もしくは＜＝400)に基づき患者を群分けし、1年間経過観察する。プライマリーエンドポイントは1年時の複合主要心血管イベント (心臓死、標的血管心筋梗塞、標的血管再血行再建)とする。よって400人の患者登録を予定する。

| Outline of Final Research Achievements |
The STRATIFY-NIRS study is a multi-center, observational study investigating the prognostic value of NIRS-IVUS-derived lipid-rich plaque among ACS patients with non-obstructive (FFR>0.80) non-culprit coronary lesions. Patient enrollment is underway in 8 Japanese centers. As of May 1st 2024, 82 patients were enrolled. We will continue patient enrollment until sample size of 400.

| Academic Significance and Societal Importance of the Research Achievements |
本研究においてFFR>0.80であってもハイリスクである冠動脈病変が存在し、その予後が有意に悪いことが証明されれば、NIRSを用いてFFR>0.80であった冠動脈病変を持つACS患者のさらなるリスク層別化が可能となり、2次予防を強化することでハイリスクプラークを持つ患者の予後をさらに改善させることが可能となる。また、本研究で得られる知見は、FFR>0.80であった冠動脈病変に対する経皮的冠動脈形成術やPCSK9阻害薬・抗炎症薬などの薬物療法の効果を評価するランダム化比較試験を計画する上での貴重な基礎データとして、患者・循環器内科のコミュニティにとって非常に有意義なものとなると考えられる。
