血中イソフラボン濃度の前立腺癌発症・増殖に与える影響に関する予防疫学的研究

Research Abstract

本研究の目的は、血中のイソフラボン濃度と前立腺癌発症リスクの関係について検証するために、症例対照研究と前向きコホート研究が計画された。症例対照研究は、検診の継続受診中に発見された前立腺癌症例を症例群とし、症例群の過去の癌を疑わない時点(PSA値が基準値以下)での年齢とPSA基礎値をマッチさせた非癌症例をコントロール群として抽出し、PSA基礎値測定時の検診受診時の血中イソフラボン濃度を保存血清を用いて測定し、血中のイソフラボン濃度とその後の前立腺癌発症との関連性を検証した。初回検診受診時の臨床的特徴は、PSA基礎値と年齢の中央値は癌症例群とコントロール群ともにそれぞれ2.17ng/ml、70歳で、初診時から最終受診時までの平均経過年数は癌症例群は4.6年、コントロール群は4.4年であった。最終受診時のPSA値(中央値)は癌症例群で5.58ng/ml、コントロール群で2.22ng/mlであった。初回検診受診時の血中Genistein、Daidzein、Equol、Glycitein濃度(ng/ml;平均±S.D.)は癌症例群は131.0±108.2、50.9±51.5、17.9±51.2、3.4±3.5で、コントロール群は194.3±258.7、101.8±113.8、20.3±34.6、7.1±7.2であった。Mann-Whitney U検定における両群間の初回受診時のイソフラポン濃度の有意差はGenistein(p=0.3232)、Daidzein(p=0.0447)、Equol(p=0.3260)、Glycitein(p=0.0487)と有意差を持って、癌症例群よりコントロール群におけるDaidzeinとGlycitein濃度が高かった。Japanese PublicHealth Center (JPHC) studyの結果では、血漿中のGenistein濃度、Equol濃度が高いほど、その後の限局性前立腺癌症の発症リスクが有意に低下することを証明したが、人種差や家族例を超える前立腺癌罹患危険因子であるPSA基礎値を症例群とコントロール群の間で適合させていない点が問題であり、またコントロール群についてJPHC studyでは臨床的に癌の診断がなされていないことを設定基準にしており、臨床癌へと進行する危険性が高い対象が含まれている可能性があり理想的な研究とはいえないが、本研究では膨大な長期間の転帰が判明しているデータベースから理想的な症例抽出をおこない、DaidzeinとGlyciteinの前立腺癌進展抑制効果が示唆されたことの意義は大きい。
