Outline of Research at the Start

本課題では、まずオルガノイドをデザインして任意の機能を持たせることができるのかという課題に挑戦する。具体的には、腸上皮オルガノイドをアルブミン分泌細胞などの肝細胞、あるいはインスリン分泌細胞などの膵頭細胞へと誘導する方法を探索する。また、このようにデザインしたオルガノイドを大腸に移植することで、機能性臓器を異所性に作成し、肝不全モデルや糖尿病モデルで治療的な効果を発揮するか否かまでを解析し、新しい再生医療のあり方を検証する。

Outline of Annual Research Achievements

本課題は、申請者が従来取り組んできた炎症性腸疾患の難治性潰瘍を腸上皮幹細胞オルガノイドで被覆し、その完全な治癒を目指す細胞治療を推進する研究を基に、腸から樹立したオルガノイドを自在にデザインし、肝不全・糖尿病などの全身的代謝疾患の治療・予防に有効な新しいデザイン化オルガノイド『ArtDO』(Artificial Designed Organoids)を開発し、それを腸へ移植することで異所性の機能性臓器の個体内での創出を目指すArtDO構想を推進し、全身性代謝異常やホルモンネトワーク異常の是正による代謝疾患の新しい治療措置の樹立を目指す課題である。
2024年度には、これまでの研究期間で樹立した、腸幹細胞オルガノイドを吸収上皮系の細胞に誘導する方法に立脚し、これを肝細胞に誘導したHI細胞（Hepato-Induction）をベースに、デザインした肝型オルガノイドの機能アッセイを予定通りに行った。同時に、吸収上皮細胞から肝細胞への誘導に関して、これをブロックする腸上皮特異的な転写因子の存在をBioinformaticsを駆使して同定し、その阻害による一層の幹細胞性獲得を検証した。
また膵内分泌ホルモンであるインスリン分泌機構に着目し、インスリノーマのインスリン異常分泌機構の解明から、インスリン分泌機構を制御する新たな因子を探索することを目的とし解析を続けている。昨年度同様インスリノーマを含む膵GEP-NENオルガノイド、手術検体を中心とした、バイオバンクを継続して樹立している。また同バイオバンクを用いてRNA-seqを行いインスリノーマ特異的に発現が上昇している候補遺伝子をいくつか同定した。その中からqPCR、手術検体を用いた免疫染色からDOCK10というインスリノーマのインスリン異常分泌を制御する新たな候補遺伝子を同定した。
これら一連の成果は、国内外での学会発表のほか、論文投稿の予定である。

Current Status of Research Progress

Reason

腸上皮細胞を肝細胞に誘導するArtDO-liverにおいても、腸上皮細胞を膵島b細胞へ誘導するArtDO-Pancreasにおいても、研究計画に記載した通りに実験が進捗しており、概ね計画通りに進行している。これらの成果はISSCR/SCSS共催のシンポジウムでの口演のほか、JST, EMBO共催のワークショップ、あるいは日本消化器病学会など多くの学会で進捗を発表しており、一部の内容については論文投稿予定である。またインスリノーマのインスリン過剰分泌機構に着目し新規制御遺伝子としてDOCK10を発見した成果は現在投稿中である。またヒト腸上皮オルガノイドを分泌系の細胞に誘導するなど分泌機構に着目したパネート細胞に関し付随して得られた成果も現在revise中となっている。

Strategy for Future Research Activity

ArtDO-liverにおいては、2024年度に樹立したHI細胞のより一層の肝細胞性の確認をIn Vitroで進めていくとともに、In Vivoでの機能評価として肝臓への移植を進める予定である。ArtDO-Pancreasにおいては、RNAシーケンスなどの手法で、樹立したバイオバンクの解析を進めるとともに今後同遺伝子がいかにインスリン分泌機構に関わっているのか、機能解析を行うとともに、ヒト腸上皮オルガノイドへのDOCK10遺伝子の導入も検討する予定となっている。
本課題では今後の最終2年間で、ArtDO移植による臨床指標改善のエビデンス獲得を目指す予定である。オルガノイド移植の方法に関しては、著者らはDSS腸炎をベースとした移植とEDTAによる大腸上皮剥離をベースとした移植の双方を自在に実施可能であり、オルガノイドの性状や肝不全・糖尿病モデルとの整合性を検討の上、より適した方法で行う予定である。
