| Outline of Research at the Start |
本研究は、地域医療の形成過程を「患者」「医療提供者」双方の視点から明らかにし、現代における地域医療の在り方を提言することを目的とする。申請者らはこれまで地域医療に関する歴史資料の所在と内容の確認を進めてきた。研究では、岐阜県大垣市に伝来する「小寺家文書」と愛知県新城市に残る「信玄病院資料」を中心に、「患者」と「医療提供者」が西洋医学受容に如何に寄与し、近代地域医療を形成してきたかを読み解いていく。得られた知見は患者教育プログラムの構築に応用する。併せて明治期の医療関係資料の整理・収集・統合を継続し、社会還元として、貴重な医療遺産を後世に伝えていくためのアーカイブ化、市民に向けた公表を行う。

| Outline of Final Research Achievements |
This study aims to clarify the formation process of community healthcare from both patient and healthcare provider perspectives, and to propose its direction in modern times. The present authors so far have confirmed the location of historical documents concerning community healthcare and its content, and found Japan’s oldest statement of inpatient medical expenses receipt.
We reexamined and analyzed the “Kodera Family Documents”including it, and we collected, investigated and organized the documents concerning the modern community healthcare. We integrated these documents analyzing how patients and healthcare providers contributed to the acceptance of Western medicine and developed the modern community healthcare. Moreover, we archived the valuable medical heritage to pass it down the generations, and made a public announcement to the citizens.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、東海地方に伝わる明治期の医療関連資料を統合するとともに、「患者」「医療提供者」双方の視点から、小寺家文書や明治の調査資料を中心として、近代地域医療の形成過程を明らかにすることを主な目的として遂行した。その結果、西濃地域では明治後期には西洋医学の定着が見られ、吉益病院、大野病院、在村医などで構成された地域医療ネットワークができつつあったことが明らかとなった。本研究は災害医療（濃尾地震）に従事し、地域医療発展に貢献した吉益雄太郎医師の存在も明らかとなり、その来歴についても把握することができた。
