Outline of Research at the Start

抗腫瘍免疫とAutophagyの関連が近年報告されているが、癌細胞のAutophagy抑制することによる腫瘍抗原の発現と樹状細胞の関係や、癌間質におけるAutophagy抑制の抗腫瘍免疫への影響はいまだ明らかにされていない。本課題では、膵癌微小環境におけるAutophagyと腫瘍免疫の関係に着目し、「膵癌細胞のAutophagyと樹状細胞の関係」「CAFのAutophagy抑制による膵癌微小環境と腫瘍免疫への影響」の2つにフォーカスしてAutophagy抑制剤と腫瘍免疫療法との併用効果を検証し、停滞する膵癌治療においてブレイクスルーとなり得る新規治療法の開発を目指す。

Outline of Final Research Achievements

This study revealed that autophagy contributes to immune evasion in pancreatic cancer by suppressing antigen presentation and shaping an immunosuppressive tumor microenvironment. Inhibiting autophagy enhanced dendritic cell activation and CD8+T cell mediated antitumor immunity. However, it also induced T cell exhaustion via LAG-3 upregulation. A triple combination therapy using chloroquine, Flt3 ligand, and anti-LAG-3 antibody overcame this effect and significantly suppressed tumor growth. Additionally, autophagy inhibition in cancer-associated fibroblasts promoted immune cell infiltration by remodeling the stroma. These findings highlight autophagy as a therapeutic target to improve immunotherapy efficacy in pancreatic cancer.

Academic Significance and Societal Importance of the Research Achievements

本研究は、膵癌におけるオートファジーが腫瘍免疫抑制に関与することを明らかにし、膵癌細胞および癌関連線維芽細胞の両面から免疫環境を制御するという新たな視点を提示した。さらに、オートファジー阻害と免疫療法の併用により、膵癌の免疫感受性を高める治療戦略の可能性を示した点で学術的に重要である。膵癌は治療抵抗性が高く予後不良な疾患であり、本研究成果は新たな治療選択肢の創出につながることから、臨床応用に向けた社会的意義も大きい。
