Outline of Research at the Start

大腸がんはマイクロサテライト不安定性の有無や遺伝子変異、体細胞染色体コピー数変化などの分子生物学的特徴からConsensus Molecular Subtypes(CMS) 1~4に分類される。近年、大腸癌において発癌と腸内細菌叢(microbiome)の関連性が報告されるようになったが、これまでmicrobiomeとCMS分類との関連について検討した報告はない。Microbiomeの解析から、便検体を用いた大腸癌のsubtype分類を可能にし、さらにmicrobiomeによってsubtypingされた腫瘍に対応する腫瘍微小環境を単一細胞レベルで検討し、個別化治療法の開発につなげる。

Outline of Final Research Achievements

We analyzed the gut microbiota from stool samples of colorectal cancer patients and examined its association with the Consensus Molecular Subtypes (CMS), but no statistically significant differences were observed. On the other hand, scRNA-seq analysis using tumor tissues revealed diverse immune cell and fibroblast populations, as well as characteristic patterns of cell-cell interactions depending on disease background. Furthermore, we reported in English journals that tumor-promoting changes were already present at the precancerous stage in colorectal cancer associated with familial adenomatous polyposis (Cancer Lett. 2024), and that T cell gene expression was altered toward a tumor-promoting immune phenotype in post-kidney transplantation colorectal cancer (Cancer Sci. 2024).
Further accumulation of clinical cases will be necessary to more thoroughly investigate the relationship between the gut microbiome and CMS classification.

Academic Significance and Societal Importance of the Research Achievements

本研究の成果は十分とは言えないものの、一定の進展が得られた。本研究結果からは、大腸癌のCMS分類を含む背景因子が、前癌病変の段階から腫瘍免疫に変化を引き起こしている可能性が示唆された。今後さらに研究を進めることで、microbiomeを標的とした新たな治療戦略の開発や、便検体を用いた非侵襲的な予後予測マーカーの確立へとつながる可能性がある。
