Outline of Research at the Start

申請者らは、日常生活下の情報に関して、信頼性の高い取得法であるEcological Momentary Assessment (EMA)や、EMAで得られたデータをリアルタイムに解析し、その場で治療介入を行うEcological Momentary Intervention(EMI)の研究を行ってきた。本研究では、それらをさらに進め、肥満症患者の生活習慣の行動変容のため、認知行動療法の技法を用いた機械学習によるスマートフォンの治療用システムの開発を行うことを目的として、以下を実施する。
(1) 治療介入用アプリケーションの開発
(2) パイロット研究
(3) ランダム化比較試験。

Outline of Final Research Achievements

While cognitive behavioral therapy (CBT) is recommended as a non-pharmacological treatment for obesity, its implementation is limited by the availability of qualified healthcare facilities. We have developed a therapeutic support smartphone application incorporating reinforcement learning algorithms to enhance behavioral goal achievement in obesity CBT. Twenty patients participated in this four-week study, with all completing the intervention period. The median BMI was 34.9 kg/m^2. Application utilization averaged 94.5%, and BMI significantly decreased from pre- to post-intervention (p=0.011). These findings suggest that our reinforcement learning-enhanced application demonstrates sufficient feasibility and may be effective for weight reduction in obese patients, offering an accessible alternative to traditional CBT delivery.

Academic Significance and Societal Importance of the Research Achievements

肥満症の非薬物療法として、認知行動療法が推奨されているが、実施可能な医療機関が限られているため、代替となる簡便な方法が求められている。本研究では、認知行動療法の専門家が不在でも、肥満症の認知行動療法における行動目標の促進に関して、治療補助となるアプリケーションを開発し、20名の患者で実施可能性と有効性を示したことで、より多くの肥満症患者へ認知行動療法を提供できる可能性を示すことができた点で、学術的意義に加えて社会的意義を示すことができた。
