Outline of Research at the Start

1) CWA-APTT、PRPを用いたCWA-sTF/FIXa、CWA-TTなどの測定系を確立する。
2) 上記の系を用いて、トロンビンバースト現象の存在・役割・関連因子などを評価する。
3) PRPとPPPの比較により、凝固系への血小板の役割を評価する。
4) 出血傾向や血栓傾向などのリスクや抗凝固療法のモニターの可能性などを、CWA-APTT、CWA-sTF/FIXa、CWA-TTなどにより 評価する。
5) sCLEC-2の測定により、血栓性疾患の血小板の活性化を評価し、抗血小板剤のモニターに使用できないか検討する。

Outline of Annual Research Achievements

・播種性血管内凝固(DIC)診断に、super formula sCLEC-2 (soluble C-type lectin-like receptor 2 (sCLEC-2)x D-dimer/血小板数が、厚生省DIC診断基準によるDICと最も有意に一致した(Diagnostics, Basel, 2023; 13: 2299)。
・急性脳梗塞の診断にはsCLEC-2 x D-dimerが最も有用であり、sCLEC-2/D-dimerは心原生脳梗塞で有意に低下し、アテローム性脳梗塞で有意に増加した(Clin Appl Thromb Hemost, 2024; 30: 10760296241232858)。sCLEC-2 x D-dimerは血栓症の診断に、sCLEC-2/D-dimerは動脈性と静脈性の血栓の鑑別に、sCLEC-2 x D-dimer/血小板数は微小血栓の診断に有用であった。
・膵癌ならびに肝癌などの担癌患者の凝固波形(CWA)は、CWA-活性化部分トロンボプラスチン時間(APTT)ならびにCWA-small amount of tissue factor induced FIX activation (sTF/FIXa)のpeak height (PH)が有意に高値であり、過凝固状態の診断に有用であった。
・CWA-APTTやCWA-sTF/FIXaのPeak time (PT)xPHあるいはPH/PTは、過凝固状態や血栓症の予知に有用であった(Ann Hematol Onco, 2024, Clin Appl Thromb Hemost, 2024; in press)。過凝固状態の原因はthrombin burstによることを、CWA-thrombin time(time)により証明した。
・FVIII製剤を投与されている血友病患者では、CWA-sTF/FIXaのPT短縮、PH増加がみられ、過凝固状態が示唆された(J Clin Med, 2023; 12: 6320)。同様に、過凝固状態の原因はthrombin burstによることを、CWA-thrombin time(time)により証明した。

Reason

・脳梗塞、COVID-19、心筋梗塞例、担癌患者などの症例は充分集積できた。
・大出血例や担癌患者血栓症の発症例は、まだ症例数が足りない。
・CWA-APTT、CWA-sTF/FIXa、CWA-TT、sCLEC-2の測定法が順調に行えた。
・過去の実験実績があり、測定法が簡便で、比較的多数例を測定できた。
・Emicizumabや中和抗体などの試薬が入手できた。 ・

Strategy for Future Research Activity

・心筋梗塞や脳梗塞症例について発症時ならびに発症後の経日変化の解析を進める。
・抗血小板療法や抗凝固療法との関係を示す。
・血友病例を含めた大出血例の解析を進める。
・血小板活性化剤によるCWAへの影響を検討する。
・血栓予防への適応を検討する。
