Outline of Research at the Start

呼吸性移動を伴う腫瘍に対して動体追跡放射線治療技術を開発し,臨床応用してきた.しかし,本治療は,正常臓器の生理機能を考慮しておらず,副作用への対応ができていない.また,呼吸による腫瘍の回転運動が,治療精度を低減させるという問題がある.本研究では,正常臓器の機能と腫瘍の回転運動に対応した次世代の動体追跡放射線治療の実現のため,バイオメカニカルモデルを用いた肺機能推定イメージング技術とロボット技術を用いた肺腫瘍回転補償寝台システムの開発を行う.

Outline of Annual Research Achievements

我々は,呼吸性移動を伴う腫瘍に対して動体追跡放射線治療技術を開発し,臨床応用してきた.しかし,本治療は,正常臓器の生理機能を考慮できておらず,副作用への対応が懸念材料となる.そこで,本年度の研究では,四次元CT画像と有限要素法を用いた患者個別の肺機能予測のための肺ストレスマップを開発した.
4次元CT(Computed tomography)画像の吸気・呼気の肺構造を有限要素法を用いて,メッシュ構造へ分解する.吸気CTに対して,CT値からヤング率を割り当てることで,初期値を決定した.吸気・呼気の肺メッシュ構造を非線形モデルを用いて,位置合わせを行い,呼吸により肺全体に加わるストレス分布を算出するアルゴリズムを開発した
開発したアルゴリズムを使用し,放射線治療開始前,治療中に撮影された4次元CTからストレス分布を算出して,放射線治療中の肺ストレス分布の変化を定量化することを可能とした.また,この変化量は,放射線肺臓炎のグレードと相関があることを示した.
バイオメカニカルモデルに基づく肺ストレス値の変化量は,従来の放射線が肺に照射される体積の指標と比較し,放射線肺臓炎を予測する精度が高いことを示し,バイオマーカーとなる可能性を示唆した..

Reason

肺腫瘍に対する放射線治療計画時に撮影される四次元CT画像に対して,医用画像処理や生体工学シミュレーションを用いることで,肺のストレス値を計算するアルゴリズムの開発をすることができた.
さらに,放射線治療時に,その変化量を捉えることで,放射線肺臓炎の発症予測の可能性について示唆した.開発したアルゴリズムの臨床応用可能性について検討することができた.

Strategy for Future Research Activity

呼吸性移動により生じる回転運動を保証するロボット機構の基盤を構築し,さらにロボット機構の検証方法を開発して行く予定である.
