胃癌は日本人において非常に罹患率の高い疾患です。手術がメインの治療方法になりますが、発見時に手術不適合の患者がいます。その場合、化学療法が選択されます。 しかしながら化学療法ができない症例、出血や通過障害のため生活の質が下がっている状態、このような場合に局所治療として放射線治療が奏効する場合があります。今後、高齢化にしたがいこのような症例は増加してくると考えられます。 これまでに止血照射野有効性は認められましたが、予後の予測およびその予測因子まで解析した研究は無いです。この研究はそれを突き止めるための研究になります。

| Outline of Final Research Achievements |
gastrointestinal-adjacent organs such as the stomach, pancreas, and liver. ① Regarding hemostatic radiotherapy for gastric cancer, there had been insufficient evidence to date. However, our prospective study demonstrated a high hemostatic response rate of 80% (Tanaka et al., 2020). Additionally, we identified an alternative method for evaluating treatment efficacy using MRI instead of endoscopy, offering a less invasive follow-up option for patients. ② In radiation therapy for pancreatic cancer, gastric irradiation is a concern due to the proximity of the stomach. Our findings revealed that the risk of radiation-induced gastritis varies depending on the stomach's condition, with fasting state reducing gastric exposure and potentially mitigating adverse effects.

| Academic Significance and Societal Importance of the Research Achievements |
これまで緩和ケアの範疇にとどまっていた末期胃癌に対し、放射線治療という非侵襲的手段によって出血などの重篤な症状を効果的に緩和できることを実証し、患者の生活の質(QoL)を大きく向上させる道を切り拓いた点にある。とくに、奏効率80%という高い止血効果と、内視鏡を用いないMRIによるフォローアップ技術は、治療の安全性と患者負担の軽減を両立させており、高齢者や全身状態の不良な患者にとっても有用な選択肢となる。これまで治療困難とされてきた部位への対応可能性が拡大した。本研究は、終末期がん患者の尊厳を保ちつつ、実際的かつ効果的な医療提供を可能にするという観点から、極めて重要な意義を有する。
