Outline of Research at the Start

本研究では65歳以上の頭頸部癌、食道癌を対象に、治療前の臨床データ、Radiomicsの手法を用いて得られた画像データの特徴量、治療情報と治療後の有害事象および治療効果データを機械学習させ、個々の高齢がん患者に対して最適な治療方針を予測するシステムの開発を行う。収集したデータを機械学習させ、各々の予測因子に対して機械学習により求めた係数をかけ予測モデル(有害事象 のリスクスコア、治療効果スコア)を構築する。

Outline of Final Research Achievements

Based on data from patients with esophageal cancer who underwent definitive radiotherapy, we constructed a Clinical model and four types of Radiomics models (tumor, muscle, muscle/lung, and muscle/lung/tumor). Among these, the Radiomics model incorporating all three-tumor, skeletal muscle, and lungs-demonstrated the highest predictive accuracy. These findings suggest that incorporating normal organ analysis is important when developing prognostic models for elderly patients. Furthermore, in the Clinical model, only the disease stage was selected as a prognostic factor, while treatment-related variables (e.g., use of concurrent chemotherapy, irradiation field) were not. This may indicate that treatment strategies are already being appropriately selected based on individual patient and tumor characteristics.

Academic Significance and Societal Importance of the Research Achievements

これまでに、腫瘍領域・正常臓器の両者を解析した予後予測モデルは報告されておらず、本研究結果により、高齢者においては予後予測モデル作成の際に、正常臓器を解析に加えることの重要性が示された。これは、放射線治療後の高齢がん患者においては運動器機能や呼吸機能の維持が予後の改善に寄与する可能性を示唆しており、どのような治療を行うかとともに、どのようにフォローし、患者ケアを行うかの個別化も必要となることを示している。
