Outline of Research at the Start

新生児医療領域においてHIEは脳障害の主な原因であり、大部分は仮死に続発し重症HIEは出生1,000 人に対して2~4人と推定されている。現在はその予後改善目的に軽度低体温療法(34度前後、72時間)を施行することが推奨されるが、その効果は限定的であり、経過中の脳浮腫が大きな予後不良因子となっている。このため本研究の目的は、新規6波長TRSを用いた脳水分量測定を行うため基礎的検討を行い、HIEにおける脳浮腫の経過と予後の関係を検討し、さらに脳浮腫軽減効果が期待される水素吸入の効果を検討する。

Outline of Annual Research Achievements

【背景】新生児低酸素性虚血性脳症(HIE)は、軽度低体温療法を行ってもその約半数が予後不良のままある。更なる予後改善のためには、新たな脳保護治療の開始が重要である。
【目的】新生仔豚仮死モデルで、低酸素虚血(HI)負荷後に水素吸入を行い、その脳保護効果をは検討した。
【対象と方法】生後24時間以内の新生仔豚を用いて、吸入酸素濃度(FiO2)を4%前後に減少させることでHI負荷を行った。負荷後24時間に継続して、無治療、軽度低体温療法、水素吸入療法、軽度低体温と水素吸入併用療法を行い、負荷後5日目に脳病理組織学的障害をスコア評価した。特に脳血管障害の有無の検討を目的に、脳組織のアルブミン染色を行った。
【結果】4群の比較で水素吸入群で血管外へのアルブミン漏出が少ない傾向を認めたが、漏出部分の面積比率では、他群との統計的な有意差は認めなかった。また低体温だけの群や低体温と水素吸入併用群との比較においてもアルブミン漏出が少ない傾向が認められた。
【考察および結論】今回の検討では統計的な有意な判定は出来なかったが、水素吸入単独群で血管外へのアルブミン漏出が少なく、この結果から水素吸入が低酸素虚血による脳障害軽減作用の機序として、脳血管障害、特に血管内皮障害を予防する可能性が示された。

低酸素性虚血性脳症の病態生理の理解のため、新生仔豚を利用し低酸素虚血動物モデルを構築し、脳血液量や脳内Hb酸素飽和度の測定を行い、特に脳血液量の変 化で脳障害の予後予測が可能な事が理解出来た。その後に本モデルを利用した、新たな治療戦略の研究を継続している

Strategy for Future Research Activity

新生児豚脳障害モデルを用いて、脳循環障害や新たな治療によるその障害軽減が可能か検討する。
