Outline of Research at the Start

本研究では、ヒト冠動脈内皮細胞 (HCAEC)とLCWE川崎病モデルマウス (C57BL/6J)を用い、川崎病における活性化HMGB1の 1) 放出阻害(ステロイド製剤やアスピリン以外の抗炎症薬(COX-2阻害薬))、2) 中和・吸着(HAT阻害薬 (Anacardic Acid、Garcinol) やトロンボモジュリン)、3) 受容体ブロック(ヘパリン)、を枢軸とした宿主自然免疫応答制御ついて、分子生物学的機序を明らかにし、新たな治療戦略を開発するための基盤となる重要な研究である。

Outline of Annual Research Achievements

HMGB1を枢軸としたDAMPsの制御は、川崎病冠動脈病変を減らす新たな治療戦略となるか検証する。【in vitro】ヒト臍帯静脈内皮細胞(HUVEC)を未処理群、siHMGB1群、s-RNA(NC)群に分け、LPS/TNF-αで刺激を行ったところ、siHMGB1でノックダウンしたHUVECでは、IL-1βやTNFαなどの炎症性サイトカインをはじめ、TLR4/RAGEなどのシグナル伝達受容体のmRNAの発現量も減少した。血管新生の評価では未処理群、siRNA(NC)群では正常な血管構造が障害され、siHMGB1群ではTNFα刺激後に血管構造の構築がみられた。【in vivo】5週齢の雄マウスにLactobacillus casei cell wall extract (LCWE) を腹腔内投与し川崎病モデルマウスを作成した。川崎病モデルマウス群を無治療群 (PBS群)、免疫グロブリン(IG)群、IG+抗HMGB1抗体 (aHMGB1) 群の3群に分類し、マウス心臓切片の病理組織、心臓組織中のサイトカインmRNA発現量を比較した。病理組織ではIG群およびIG+aHMGB1群は無治療群と比較して、冠動脈周囲のマクロファージの浸潤、外弾性板の断裂、中膜平滑筋層の増生が抑制された。組織サイトカインmRNA解析は、無治療群でIL-1β、TNFαなどの発現量が増加したが、IG群およびIG+aHMGB1群ではこれらの発現量が低下した。さらに、IG+aHMGB1群はIG群と比較しMCP-1を抑制し、IL-1β、IL-6などの炎症性サイトカインの発現を減少させることが分かった。本研究成果から、免疫グロブリンに抗HMGB1抗体を併用することで、冠動脈周囲のマクロファージの浸潤を抑制し炎症性のサイトカイン・ケモカインやHMGB1シグナル伝達を抑え、冠動脈後遺症発症を減少させる可能性が示唆された。

in vivoでの実験成果を再確認する目的で、in vitroでの実験を複数回にわたり実施した。検証を重ねているため、時間を要している。

Strategy for Future Research Activity

私たちは、川崎病冠動脈病変を抑え遠隔期の心後遺症を減らすため、HMGB1を枢軸とした宿主自然免応答の制御機構が重要と考え、新規治療薬の開発に有望と考えている。①活性化HMGB1の放出阻害、②HMGB1の中和・吸着、③HMGB1受容体のブロック、を念頭に、①ステロイド製剤や抗炎症薬、②ヒストンアセチル化酵素(HAT)阻害薬やトロンボモジュリン (TM)、③ヘパリンやヘパラン硫酸鎖ならびにその構造誘導体、に着目した。これらの薬物が宿主自然免応答制御の新たな治療薬候補となると仮説を立て、検証していく。
