Outline of Research at the Start

自己免疫性脳炎の治療水準向上のために、抗神経抗体同定方法の確立、抗体産生細胞の分化と浸潤の機序解明、抗神経抗体による神経障害の機序解明という三つの課題を設定し、人対象臨床研究と病態解明研究を行う。多施設共同研究により集積した臨床試料を使用し、cell-based assayにより抗神経抗体を同定するとともに測定法を最適化する。またB細胞関連液性因子の網羅的解析により、抗神経抗体産生の背景となる免疫学的異常を明らかにする。さらに同定された抗神経抗体を培養神経細胞やマウスに投与してシナプスやマウス脳病理・行動に及ぼす影響を観察し、抗体が神経症状や神経学的後遺症をもたらす機序を解明する。

Outline of Annual Research Achievements

多機関共同研究による症例・試料の収集を継続し、新たにレジストリに116症例、バイオレポジトリに251試料を登録した。
なお「人を対象とする生命科学・医学系研究に関する倫理指針」の一部改正等に合わせて研究体制の見直しを行なった。具体的には症例レジストリを従来の紙ベースからWebベースに変更し、共同研究者等が臨床情報をクラウド上データベースに直接入力する方法とした。新たなデータベースは「小児炎症・免疫性神経疾患症例登録システム」と命名し、厚生労働科学研究・難治性疾患政策研究事業や難病プラットフォームRADDAR-Jと連携しながら、データの二次利用も可能な体制を構築した。また電磁的説明同意を新たに導入した。
レジストリのデータを用いて小児のNMDA受容体脳炎(NMDARE)のprobable診断基準に関する検討を行なった。NMDAREに対しては成人においてNMDAR抗体陽性を予測するprobable臨床診断基準が作成され広く用いられている。我々は先行研究においてこの基準が小児にも適用できるかどうかを検討し、感度は十分であるものの特異度に問題があることを報告した。そこで統計学的手法を用いて小児にも適用できる基準の作成を試みた。小児自己免疫性脳炎の症例(264例)をNMDAR抗体陽性群(39例)と陰性群(225例)二分、陽性群で有意に多くみられる14の臨床的特徴を抽出した。これらの14項目に重みづけを行い、小児用のNMDARE予測スコアを作成した。新たなスコアは感度・特異度ともに80%を超え、小児NMDAREの予測に有用であることが明らかになった。NMDAR抗体の測定は我が国では保険収載されておらず臨床診断への応用が進んでいないことから、本研究の成果はより迅速かつ確実なNMDARE診断に役立つと期待される。

Strategy for Future Research Activity
