Outline of Research at the Start

循環器分野はデバイス医療の進歩とともに発展を遂げた。動的循環制御システムの再構築において必須である「心室補助」、「脈調節」、「自律神経調節」においては、循環補助装置、心臓ペースメーカー、神経電気刺激装置がそれぞれ開発されている一方で、独立した開発過程をたどり、動的循環制御システムにまで介入ができない。既存機器に対して、動的循環制御を目指した制御アルゴリズム構築と機器同士の動作統合を行うことで、経験をベースとした医療を超えた未来医療が可能となる。本研究では、急性心筋梗塞の心筋ダメージと急性心不全時の血行動態破綻にターゲットを絞り、革新的循環器疾患治療機器開発につながる基盤研究を推進する。

Outline of Final Research Achievements

This research aimed to establish a proof-of-concept (POC) for an innovative treatment strategy by integrating multiple medical devices to fundamentally improve circulatory dynamics. Prior to developing integrated control technology, we conducted simulation studies on percutaneous left ventricular assist devices and published several papers based on these findings. We also investigated autonomic cardiovascular regulation using large animal models. Furthermore, we developed control algorithms to comprehensively optimize cardiac load and autonomic regulation, and subsequently published our results.

Academic Significance and Societal Importance of the Research Achievements

心筋梗塞や急性心不全において心臓への負荷を下げ、血行動態を安定化させることは、病態生理上明らかな短期ゴールである。一方、劇的に心血管機能が変化する中で、それらを動的に達成するために、専門医や専門スタッフが経験と知識を駆使し、複数のデバイス操作を行っている現状がある。循環器デバイスの多くは電気で動作し、電気信号から動作までの速度は秒以内で調節可能である。また、多くのデバイスによる心血管の応答も秒単位である。申請者らが培った循環制御技術を応用し、デバイス同士および生体とデバイスが最適かつ動的に組み合わされることで、専門医と同等もしくはそれを超えた介入・治療が可能になる
