Outline of Research at the Start

冠動脈硬化診断法としてMRI・冠動脈CTにおいて臨床転帰を規定する画像上の特徴的な指標がある一方、人工知能・機械学習が導入されている中で、視覚のみに頼っていた従来の画像診断指標(特徴量)をRadiomics技術で数値化することにより、疾病の重症度をより高い精度で診断できる有力な候補になりうる。さらに、Radiomics技術に遺伝子情報(Radiogenomics)や蛋白質発現 (Radioproteomics)といった生化学情報を付加し画像情報を補完することで精度の高い予後予測・および治療標的の解明につながる高精度な心臓事故予測技術を開発する。

Outline of Final Research Achievements

High-risk coronary plaques identified on coronary T1-weighted imaging (T1WI), defined as having a plaque to myocardial signal intensity ratio (PMR) of 1.4 or more, were found to be independent predictors of reduced fractional flow reserve (FFR), regardless of the degree of stenosis. A PMR value of 1.1 was identified as the cutoff for predicting FFR of 0.80 or less. A machine learning model using coronary T1WI to detect high-risk plaques achieved an area under the curve (AUC) of approximately 0.80. Furthermore, comparisons between T1WI findings and autopsy specimens, along with proteomic analysis, revealed distinct protein expression patterns corresponding to plaque risk levels.

Academic Significance and Societal Importance of the Research Achievements

本研究によりT1強調MRIによる詳細な冠動脈リスク層別化が可能となったのみならず、その生化学的背景を明らかにすることができた。生化学的背景の解析結果から今後の冠動脈硬化予防法の開発につながることが期待される。
