Outline of Research at the Start

本研究の目的は深層学習を活用した多面的な病理学的検討によって、小児期発症肺高血圧症における予後規定因子を明らかにすることである。一部の肺高血圧症については治療薬が開発され、予後は大きく改善したが、死亡や肺移植に至る患者がいまだ多数存在する。そして小児では乳幼児期に急速に肺高血圧が進行し、各種治療に反応しない重症例が散見される。本研究では重症化に寄与しうる要因の発生機序や遺伝学的背景、臨床像への影響等を後方視的に調査し、また、新たな観点から見直した肺病理所見を統合し、さらに人工知能を駆使した解析を行うことで、小児期発症PHの病態を解明し、一人一人の患者に最適な治療を実施するための指標を創出する。

Outline of Final Research Achievements

At the Japanese Research Institute of Pulmonary Vasculature, lung tissue specimens from pediatric patients diagnosed with pulmonary hypertension were stored and selected as study subjects. Subsequently, the attending physicians of each subject were contacted to conduct an investigation into their clinical profiles. Initially, the study focused on patients with pulmonary hypertension associated with congenital heart disease, and analyses were conducted. As a result, we demonstrated that the Index of Pulmonary Vascular Disease (IPVD), a proprietary evaluation index developed by the Japanese Research Institute of Pulmonary Vasculature to assess pulmonary arterial lesions in pulmonary hypertension, is a useful predictor of long-term prognosis. These findings were published in a peer-reviewed journal. Currently, we are linking lung tissue preparations with clinical data and continuing to search for prognostic factors other than IPVD, using artificial intelligence.

Academic Significance and Societal Importance of the Research Achievements

本研究は先天性心疾患に伴う小児期発症肺高血圧症において、従来のHeath-Edwards分類と比較して、IPVDがより広範囲にわたる肺動脈の病変を評価し、かつより精度の高い予後予測を可能にすることを明らかにした。この結果は、肺高血圧症における予後評価における新たな視座を与えるものであり、今後の臨床現場での診断や治療方針における有益な指針となり、さらには患者の予後改善に寄与することが期待される。
