Outline of Research at the Start

肥満は糖尿病や高血圧、高脂血症などの代謝性疾患の上流に位置している。したがって、肥満症の治療はこれら代謝性疾患とその下流にあるすべての関連疾患を予防・改善できる可能性があり、医療経済学的観点からも重要である。NKT(ナチュラルキラーT)細胞は、肥満に伴う慢性炎症の誘導に重要な働きをしている。脂肪細胞は脂肪組織におけるNKT細胞の活性化に寄与しているが、どのような性質をもつ脂肪細胞が機能しているかは未だわかっていない。本研究では、脂肪細胞によるNKT細胞の活性化機構、特に脂肪細胞の脂質蓄積に着目し、脂肪細胞の変化に伴うNKT細胞への影響を検討する。

Outline of Annual Research Achievements

変形性膝関節症(OA)患者の手術検体より膝蓋下脂肪体を採取し、組織に浸潤するリンパ球をフローサイトメーターで解析したところ、CD4陽性T細胞やCD8陽性T細胞の他、MAIT細胞が多く確認できた。また、MAIT細胞におけるサイトカイン産生を調べると、TNF-aやIFN-gの発現を確認できた。それに比べて、IL-17の発現量は少ないことがわかった。加えて、MAIT細胞のエフェクター活性を示す転写因子であるRORgtやT-bet、Eomesの発現量も解析している。特に、エフェクターT細胞やMAIT細胞は肥満における脂肪組織炎症との関連が多く報告されているため、本研究では膝蓋下脂肪体におけるエフェクターT細胞やMAIT細胞と肥満について注目している。膝蓋下脂肪体におけるCD4陽性T細胞やCD8陽性T細胞、MAIT細胞の割合や、それらのエフェクター活性がOA患者によってどのような違いがあるのか、BMIや血清トリグリセリド、疼痛スコアなどの因子と相関があるかなど検討する予定ある。

Strategy for Future Research Activity

OA手術検体数を増やし、主に膝蓋下脂肪体に浸潤するT細胞(CD4陽性、CD8陽性、MAIT)の割合とそのエフェクター機能がOA患者の肥満に関わる因子(BMI、トリグリセリド、HbA1c)や疼痛スコアとどのような相互関係があるかを解析する。
