Outline of Research at the Start

肥満は糖尿病や高血圧、高脂血症などの代謝性疾患の上流に位置している。したがって、肥満症の治療はこれら代謝性疾患とその下流にあるすべての関連疾患を予防・改善できる可能性があり、医療経済学的観点からも重要である。NKT(ナチュラルキラーT)細胞は、肥満に伴う慢性炎症の誘導に重要な働きをしている。脂肪細胞は脂肪組織におけるNKT細胞の活性化に寄与しているが、どのような性質をもつ脂肪細胞が機能しているかは未だわかっていない。本研究では、脂肪細胞によるNKT細胞の活性化機構、特に脂肪細胞の脂質蓄積に着目し、脂肪細胞の変化に伴うNKT細胞への影響を検討する。

Outline of Annual Research Achievements

申請者はこれまでに、肥満・脂肪組織炎症に関わるリンパ球の機能を研究してきた。肥満マウスモデルを用いて、特にNKT細胞を中心とした脂肪組織内のリンパ球の活性化機構や細胞間相互作用を解析してきた。本研究では、ヒトの膝蓋下脂肪体を用いて、炎症と組織に浸潤するリンパ球プロファイルの関連性を検討する。変形性関節症(OA)患者の手術検体より膝蓋下脂肪体を採取し、組織に浸潤するリンパ球をフローサイトメーターで解析したところ、CD4陽性T細胞やCD8陽性T細胞、MAIT細胞の確認ができたが、NKT細胞はほとんど認められなかった。これまでに得られた検体のリンパ球プロファイルと年齢やBMI、また生化学検査項目との相関を検討した。その結果、CD4陽性T細胞の割合と血清トリグリセリド値が正の相関を示し、一方、MAIT細胞の割合と血清アルブミン値は負の相関を示すことがわかった。また、年齢と活性化MAIT細胞の割合が正の相関を示した。特に脂肪組織におけるCD4陽性T細胞の機能は研究が進んでおり、Th1細胞は肥満・脂肪組織炎症に対して促進的に、反対にTh2細胞や制御性T細胞は抑制的に働くことがわかっている。本研究結果から、CD4陽性T細胞と血清トリグリセリド値が相関することが示されたが、どのCD4陽性T細胞サブセットが相関するかを調べることが重要であると考えている。近年では、老化細胞の機能解析についての報告が増えており、年齢と活性化MAIT細胞との関係性も興味深く、老化MAIT細胞という視点で解析を進めたいと考えている。

OA手術検体数を増やし、生化学検査項目だけでなく、膝蓋下脂肪体のリンパ球プロファイルと疼痛スコアとの関連性も検討する。
