Outline of Research at the Start

Schwann細胞(SC)は神経修復過程において未熟な修復SCへと脱分化することが報告されているが、脂肪由来間葉系幹細胞(ADSC)から分化誘導したSchwann様細胞(SLC)をどこまで分化させればより修復SCに類似した細胞になるかは不明である。今回SLCの分化度に注目し、発現するタンパク質、遺伝子を解析することで作成したSLCの至適分化時間を検討する。またSLCから分泌されるexosomeのみの投与でも神経再生促進効果が得られるかどうか検討する。

Outline of Final Research Achievements

Adipose-derived mesenchymal stem cells (ADSCs) were induced to differentiate into Schwann-like cells (SLCs), and the expression of S100 was confirmed by PCR. SLCs significantly improved neurite outgrowth in DRG neuron cell lines with neuropathy. In addition, rats with urinary dysfunction were created by crushing the pelvic splanchnic nerves under general anesthesia, and cystometric measurements were performed. In the urinary dysfunction group, maximum intravesical pressure (MP) was significantly reduced on the 14th day after surgery, however MP improved in the SLC transplant group. In the urinary dysfunction group, the thickness of the bladder muscle layer and the amount of collagen fibers increased, whereas the SLC transplant group showed significant improvement. These results confirmed the nerve regeneration effect of SLC transplantation.

Academic Significance and Societal Importance of the Research Achievements

直腸癌手術における骨盤内臓神経損傷による術後排尿障害は、確立された治療法はなくQOL低下の一因となる。今回Schwann様細胞による骨盤内臓神経再生の可能性が示された。臨床応用できれば直腸癌術後の排尿障害に対する画期的な治療法となる。
