| Outline of Research at the Start |
GvHD-胆管炎モデルマウスを用いてGvHDと胆管炎との関連を明らかにし、BA患児の自己肝生存率の改善に考慮した術後管理方法、さらには早期診断および治療介入方法の開発が重要で、自己肝生存率の向上を目的とした、肝移植に依存しない新たな治療法を確立する必要があると考えた。本研究はこれまでの当科におけるMaternalmicrochimerismにより生じるGvHD論に基づいたBAの研究を礎とし、世界的にも未だ報告のないGvHD-胆管炎モデルマウスを新規開発し、GvHDを背景とした胆管炎の発症メカニズムを免疫学的に評価し、胆管炎の有無による免疫と寛容のバランスを検索することでBAに生じる胆管炎の病態を解明する。

| Outline of Final Research Achievements |
This study elucidated the pathophysiological relationship between maternal microchimerism and graft-versus-host disease (GvHD) in biliary atresia. We established a GvHD model using BALB/C and B10D2 mice to investigate the pathogenetic mechanisms underlying biliary atresia-like conditions. Our findings revealed that inflammation patterns in early postoperative cholangitis serve as important predictive indicators for long-term outcomes, particularly highlighting the crucial role of non-suppurative cholangitis in improving prognosis. Additionally, we discovered significantly enhanced lymphangiogenesis in the left lateral segment of liver biopsies obtained during Kasai portoenterostomy, demonstrating the involvement of the lymphatic system in biliary atresia pathogenesis. Our research also identified specific inflammatory markers and immune cell populations that correlate with disease progression and treatment response.

| Academic Significance and Societal Importance of the Research Achievements |
【学術的意義】本研究は胆道閉鎖症の病態機序における母児間免疫と移植片対宿主病の関連を世界で初めて解明し、リンパ管新生の重要性を発見した画期的研究である。炎症パターンによる予後予測法の確立は、小児外科学分野における新たな診断・治療指針を提示すると考える。 【社会的意義】胆道閉鎖症は新生児期に発症する難治性疾患であり、本研究成果により早期診断と個別化治療が可能となる。これにより患者の自己肝生存率向上と肝移植回避が期待され、医療費削減と患者家族のQOL改善に大きく貢献する。また、予後予測の精度向上により適切な治療選択が可能となり、社会全体の医療負担軽減にも寄与する。
