Outline of Research at the Start

本研究では「小膵癌」の診断精度を劇的に向上させるべく、画像所見(医用画像)と膵液所見(膵液中のバイオマーカー)を人工知能(AI)技術を用いて統合解析し、膵癌の発症予測・早期診断法を確立することを目的とした。連続膵液細胞診目的に採取された高純度な膵液サンプルを用いて生体内分子の統合的解析(Trans-omics)と医用画像解析(Radiomics)を総合したマルチオミックス(Multi-omics)解析を行い、小膵癌を高精度に検出しうる診断アルゴリズムを構築する。

Outline of Final Research Achievements

To improve the long-term outcomes for pancreatic cancer, biomarker analyses targeting mutated and methylated DNA using pancreatic juice samples were performed in cases with intraductal papillary mucinous neoplasm (IPMN), a kind of risk for pancreatic cancer. Mutated DNA and methylated DNA were measured in the cell-free fraction of 156 pancreatic juice samples serially collected for cytological examinations from 30 cases with IPMN. Even in juice samples with inadequate cytological diagnosis due to insufficient amounts of cellular component, mutated KRAS/GNAS DNA and methylated SOX17/TBX15 DNA were both measurable and our results of diagnostic performance of their combination can help detect noninvasive pancreatic precursor neoplasms.

Academic Significance and Societal Importance of the Research Achievements

膵液中のセルフリー分画から腫瘍由来DNAの変異解析、メチル化DNA解析が可能であったことから、膵液の細胞成分は細胞診へ、上清のセルフリー分画はバイオマーカー解析へ有用性の高い生体試料となりうることが示唆された。本研究では標的遺伝子はごく少数に限られたが、今後はmRNA、miRNAなどのDNA以外核酸を標的とした解析などへの応用が期待される。将来的には診断精度の高いバイオマーカーを複数明らかにし、それらを組み合わせて多数例での検証を行うことで、臨床実装へ向けた橋渡しとなりうる。さらには膵癌の予後向上へも期待がもたれる。
