Outline of Research at the Start

ミネラルコルチコイド受容体拮抗薬であるエサキセレノンのもつ心筋保護効果を基礎実験にて分析する。ランゲンドルフ装置を用いたラット摘出心による灌流実験を行い、全虚血に対するエサキセレノンの心筋保護効果や標準的高K心筋保護液(セントトーマス病院2号液)へ及ぼす増強効果を検証する。

Outline of Final Research Achievements

We investigated whether esaxerenone (Esax) exerts a myocardial protective effect against global ischemia using a Langendorff apparatus and a perfusion experiment using isolated hearts from Wistar rats. Esax administered before ischemia exerted a myocardial protective effect against 30 minutes of global ischemia, but no significant protective effect was observed in the group administered after ischemia. Regarding the effect of Esax on the myocardial protective effect of St Thomas' Hospital No. 2 solution (STH2), Esax administered before ischemia enhanced the myocardial protective effect against 30 minutes of STH2 cardiac arrest. We investigated whether endothelial function is involved in the enhancement of Esax's myocardial protective effect by STH2, and found that it was inhibited by L-NAME, suggesting its involvement.

Academic Significance and Societal Importance of the Research Achievements

慢性心不全に対する抗アルドステロン作用を有するミネラルコルチコイドブロッカーの有用性は広く知られている。この1つであるエサキセレノンを術中の心筋保護法に応用するべく検討を行った。エサキセレノンの虚血前投与は心筋保護効果を発揮するとともに、St Thomas'病院2号液のもつ心筋保護作用を増強する効果も認められた。今後、研究を継続し臨床応用を目指したい。
