D-ダイマー高値という疾患横断的観点で、死亡に関連する予測指標は検討されておらず、機械学習手法も活用されていない。申請者は「D-ダイマーが300 μg/mL以上の連続100例の検討」で、院外心停止を除き、72時間死亡率が30%に達することを見いだした。本研究では、罹患疾患によらず、D-ダイマー高値という俯瞰的視点で72時間死亡に着目する。その予測指標をD-ダイマー以外の臨床検査項目から、多重ロジスティック回帰分析と機械学習分析とを比較検討することで高精度の予後予測モデルを構築し、その汎化性能も検証する。検証した指標に治療介入することで、D-ダイマー高値を示す救急患者の予後の改善を目指す。

Outline of Annual Research Achievements |
平成30年(2018年)から令和元年(2019年)の2年間の臨床検査データベースを対象とした。年齢、性と臨床検査32項目のデータともに、D-ダイマーが2 μg/mL以上の11,750例を抽出した。18歳未満を除外した後に、患者IDを連結可能匿名化した。重複例は、D-ダイマー値が一番高かった検査日のデータシートだけを残し、それ以外の5,766例を除外した。 72時間転帰は、電子カルテから自動的には抽出できない。重複例を除外した5,993例の72時間転帰を、電子カルテで1例1例調査し、院外心停止だった332例を除外した。現在、残りの5,661例の転帰の確認調査を進めている。
