Outline of Research at the Start

悪性脳腫瘍の代表的疾患である神経膠腫は、変異型IDHを有するものと有さないものとに2分され、形態は類似していても遺伝学的・代謝学的背景は大きく異なる。治療薬としてテモゾロミドが使用されることが多いが、その薬剤耐性が大きな問題となっている。本研究では変異型IDH神経膠腫モデルと野生型IDH神経膠腫モデルを用いた網羅的遺伝子解析、代謝解析結果に基づいた差異が、それぞれのIDH型における治療標的とならないかをin vitroとin vivoで検証する。更にはテモゾロミド耐性となった腫瘍においても有効ながん代謝を標的とした治療法を開発し、将来の臨床応用へとつなげていく。

Outline of Final Research Achievements

Comprehensive metabolomic analyses using mutant IDH models derived from normal human astrocytes and tumorigenic wild-type IDH models revealed that asparagine levels were relatively lower in the wild-type IDH model, while glutamine and glutamate levels were lower in the mutant IDH model. L-asparaginase, which converts asparagine to aspartate, exhibited a greater antitumor effect in the wild-type IDH model. In contrast, the GLUD1 inhibitor, which converts glutamate to α-ketoglutarate, showed higher efficacy in the mutant IDH model. Similar results were observed in the experments using in vivo models. These findings suggest that targeting tumor metabolism based on IDH mutation status is a promising therapeutic strategy.

Academic Significance and Societal Importance of the Research Achievements

本研究では、IDH変異の有無に応じたグリオーマのアミノ酸代謝の違いに着目し、それぞれの代謝脆弱性を標的とした治療戦略の有効性を示した。IDH野生型ではアスパラギン枯渇を介したL-アスパラギナーゼ、IDH変異型ではGLUD1阻害が抗腫瘍効果を示した。代謝依存性を活用した新規治療法の確立に貢献し、難治性脳腫瘍に対する個別化医療の実現に向けた基盤的知見を提供する成果である。
