Outline of Research at the Start

ラットくも膜下出血後遅発性脳虚血モデルを用いて、遅発性脳血管攣縮による脳虚血が生じる前から人工酸素運搬体HemoActを経静脈的に連日投与し、1)大脳皮質局所脳血流と脳組織酸素分圧の経時的変化、2)脳内微小動脈でのHemoActの灌流状態の組織学的評価、3)神経細胞死やミクログリアが介在する炎症反応の病理組織学的評価を行い、HemoActの脳微小循環障害改善効果と脳保護効果を検証する。くも膜下出血後の遅発性脳虚血は、発症後4から14日目に生じるため、遅発性脳虚血を生じる前から投与を開始する本実験系は、実際の治療応用を想定した基礎的概念実証のための実験となる。

Outline of Final Research Achievements

During the three-year project period, we conducted a preliminary study using a rat subarachnoid hemorrhage (SAH) model (stereotactic prechiasmatic cistern autologous blood injection model) and evaluated the effects of intravenous administration of an artificial oxygen carrier, hemoglobin nanoparticles (HbNP). The results suggested that a single systemic intravenous administration of HbNP exerted anti-inflammatory effects by suppressing microglial activation in the cerebral cortex following SAH-induced cerebral ischemia. Further investigations are required to determine the potential of HbNP as a neuroprotective agent against delayed cerebral ischemia after SAH, including comparative analyses of oxidative stress markers (such as 4-Hydroxynonenal [4HNE]) expression in brain tissues between experimental groups.

Academic Significance and Societal Importance of the Research Achievements

脳動脈瘤破裂によるくも膜下出血の最終転帰を左右する遅発性脳血管攣縮は、いまだ治療法
が確立していない。現状確立している主幹動脈攣縮抑制治療では達成できない、脳微小循環改善・末梢脳組織酸素化改善作用による脳血管攣縮治療法として、今回明らかにした人工酸素運搬体HbNPの1回静脈内投与による、大脳マイクログリアの活性化を抑制する抗炎症作用が、くも膜下出血後遅発性脳虚血に対する脳保護薬として臨床応用可能な可能性がある。さらなる基礎的検討を加えることで、くも膜下出血後primary injuryを除く微小循環障害・炎症・酸化ストレスなど遅発性脳血管攣縮の様々な病態が複雑化する前の先制的介入が可能となる。
