Outline of Research at the Start

整形外科領域の手術においては、その骨癒合は受動的な自然治癒に頼っているのが現状である。申請者らは既存の薬剤とは構造やメカニズムが異なる新たな骨癒合促進物質を開発した。本研究ではハイドロジェルなどの高分子化合物との複合体を用いながら、前臨床的な動物実験(異所性骨形成モデル、骨折モデル、脊椎固定モデル)におけるこの化合物の局所注入を主体とした治療効果を検証し、骨形成促進のメカニズムについての検討も行う。

Outline of Annual Research Achievements

化合物Pはそれ単体で筋肉内で骨誘導能を呈するが生体内で吸収されやすく、効力を発揮するためには数週間は生体内で残存する必要があることが課題である。本研究ではそれを解決するために高分子化合物などとの複合体により吸収率の低下を試みた。まずゼラチン溶液に同量の化合物Pを分散させて複合体を作成した。この複合体は低温で半固体となり、局所の滞留性が非常に良くハンドリングも良好であった。これをマウス大腿骨周囲に埋植したところ、化合物Pはそれ単体の時と比べてより長期に残存した。単体では投与後2-3週程度で大部分が消失したのに対し、投与後6週まで明瞭な残存を認めたが、単体に比べて明らかな骨誘導能の増加は認めなかった。さらに剤形に工夫を加えこの化合物P含有ゼラチン溶液をゲニピンにて架橋するとその投与量に伴いより硬度の高い物質が得られたが、あまりにも生体吸収性が低かった。
これらの結果から本化合物は数週間の残存も重要であるが、相反してスムーズな吸収も骨形成に重要な因子であることが分かった。したがって過度な吸収抑制は骨形成能の低下を生じうるため、分散液としてペースト状としてコントロールを試みた。すると単体の投与に比べて効率的な骨形成が生じることが分かった。適度に遺残も認めることが奏功したと考えられるが、一部は永続的に残存してしまうことが観察されたため、今後は工夫を加えて体内残留を最小限にしたい。また骨形成に必要な投与量の最適化も行う予定である。

単純に化合物の吸収を抑えれば良いのではなく、適度に残留しつつも適度に吸収される必要があることが分かった。

Strategy for Future Research Activity

残留と吸収のバランスを持ち合わせた工夫を模索していく。化学組成だけでなく、物理的な処理によりコントロールできる可能性があると考えている。
