Outline of Research at the Start

進行性腎癌に対する癌免疫療法の適応は拡大傾向だが、奏効する症例は限定的であり、臨床応用された治療効果予測マーカーは存在しない。われわれは先行研究において、尿路上皮癌患者の血清由来EVs内のFirmicutes門発現が癌局所内免疫状態と有意に相関し、癌免疫療法の治療効果・予後予測因子となることを見出した。本研究では、腎癌患者における血液内細菌遺伝情報の発現状態が宿主に与える影響ならびに臨床的意義について評価し、癌免疫療法の治療効果予測マーカーの開発と臨床応用を目標とする。

Outline of Final Research Achievements

In serum extracellular vesicles from patients with renal cell carcinoma (RCC), Bacteroidia DNA was found to be expressed at higher levels compared to healthy controls. Analysis of the immune status in RCC tissues paired with the serum samples revealed a positive correlation between the amount of Bacteroidia DNA in the serum extracellular vesicles and the proportion of PD-1^low TIM-3^high CD4-positive T cells, a population previously identified as regulatory T cells in our prior study. Furthermore, among RCC patients treated with immune checkpoint inhibitors, those with high Bacteroidia DNA expression showed significantly poorer prognosis compared to those with low expression. These findings suggest that Bacteroidia DNA within serum extracellular vesicles could be associated with regulatory T cells in RCC tissues.

Academic Significance and Societal Importance of the Research Achievements

切除不能な進行性腎癌においては、複数の免疫チェックポイント阻害薬(ICI)の併用や、ICIと分子標的薬を組み合わせた治療が選択されるようになったが、ICIに対して治療抵抗性を示す患者が存在している。一方、免疫抑制細胞である制御性T細胞が、癌細胞に対する免疫応答を抑制して癌の進行に寄与するとの報告がある。治療抵抗性と関連する細菌情報を探索することにより、細菌由来細胞外小胞を用いた治療効果増強薬の開発につながると考える。
