Outline of Research at the Start

原発開放隅角緑内障(POAG)感受性遺伝子多型は、眼圧(IOP)上昇に関与するIOP-related遺伝子多型と、眼圧とは別に視神経の脆弱性に関与するnon-IOP-related遺伝子多型の2つに分類されると考えられる。
本研究では、IOP-related/non-IOP-related遺伝子多型相加効果と視野障害の関係について検討し、IOP-related遺伝子多型が視野重症度、びまん性視野障害、non-IOP-related遺伝子多型が傍中心暗点や鼻側階段など緑内障性(局所)視野障害、上方または中心視野障害に重要な役割を果たしているか検証する。

Outline of Final Research Achievements

The genetic variants predisposing to primary open-angle glaucoma (POAG) that can be classified into those associated with intraocular pressure (IOP) elevation (IOP-related genetic variants) and optic nerve vulnerability (optic nerve-related genetic variants) were genotyped to investigate the genetic variants associated with visual field loss in POAG. The associations between the genetic risk score (GRS) of IOP-related/optic nerve-related genetic variants and the mean deviation (MD) of automated static perimetry as an indicator of the severity of visual field loss and pattern standard deviation (PSD) as an indicator of the focal visual field loss were evaluated. There were significant associations between the IOP-related/optic nerve-related GRS and MD/PSD respectively. The IOP-related (IOP elevation) and optic nerve-related (optic nerve vulnerability) genetic variants may play an important role in the severity of visual field loss and focal glaucomatous visual field loss respectively.

Academic Significance and Societal Importance of the Research Achievements

本研究により、POAGの緑内障性視野障害メカニズムが遺伝学的により明らかとなった。これら遺伝情報の積み重ねから、将来的には緑内障性視野障害発症進行リスクを遺伝学的に評価することが可能となり、早期発見治療が重要と考えられている緑内障診療の一助となるだけでなく、視野進行リスクに合わせた目標眼圧設定、治療法選択が可能となるかもしれない。また、個々の症例の臨床像(表現型)と遺伝背景に合わせて、最適かつ有効な治療の選択といった、いわゆるオーダーメイド医療を行える可能性もあり、より質の高い医療を提供できると共に医療経済的にも有益であると考えられ、今後の緑内障治療発展の足掛かりとなった。
