二次性副甲状腺機能亢進症モデル動物(5/6腎臓摘出ラット)における骨細胞ネットワークの石灰化抑制および骨細胞性骨溶解誘導と、カルシウム受容体作動薬投与によるミネラル沈着・石灰化骨基質成熟化の機序解明を目的として、溶媒またはカルシウム受容体作動薬を投与した5/6腎臓摘出ラットを作成し、血清ミネラル濃度などの血清パラメーター、電顕解析などの微細構造解析を含む骨組織解析、シングルセルRNA-seq解析等を行うとともに、培養骨細胞株によるin vitro実験を遂行する。

| Outline of Final Research Achievements |
In this study, we conducted in vivo and in vitro analyses to elucidate the pathogenic mechanism underlying cortical porosity in secondary hyperparathyroidism associated with chronic kidney disease (CKD-SHPT). Our findings revealed that in CKD-SHPT model rats, elevated serum levels of parathyroid hormone (PTH) and inorganic phosphate (Pi) induced osteocytic osteolysis, inhibition of bone mineralization in osteocytic lacunae, and osteocyte death. These osteocytic abnormalities led to the enlargement of osteocyte lacunae, which were subsequently invaded by osteoclasts and blood vessels, thereby promoting cortical porosity. Furthermore, calcimimetics suppressed osteocytic abnormalities by reducing serum PTH levels, suggesting its potential to inhibit the progression of cortical porosity.

| Academic Significance and Societal Importance of the Research Achievements |
慢性腎臓病に伴う二次性副甲状腺機能亢進症における皮質骨多孔化は、PTHによる高骨代謝回転のみで考えられてきた。本研究は、皮質骨多孔化が血中PTHおよびPi濃度の上昇による骨細胞異常に起因することをモデル動物レベルで明らかにするとともに、カルシウム受容体作動薬が皮質骨多孔化の改善に寄与することを見出した。これらの研究成果は、基礎的な骨代謝学の理解のみならず、同疾患の治療法や予防法の開発にもつながると考えられ、高い学術的・社会的意義を有すると思われる。
