Outline of Research at the Start

本研究では,口腔運動の巧緻性の向上には,体性感覚や視覚等の感覚情報入力の制御が重要であることに着目し,運動機能改善プロセスにおける,運動遂行の巧緻性に対する感覚入力制御の効果,および能動的触知覚(アクティブタッチ)概念の口腔への応用を検証する.更に,口腔感覚運動と嚥下運動の機能連関メカニズム解明へと新たに展開させ,摂食嚥下障害における口腔機能低下への対応を神経生理学的に実証することを目的とする.

Outline of Final Research Achievements

The present project investigated how control and modification of sensory information input during tongue movement can improve and modulate the accuracy of movement.
When maintaining strong tongue pressure with visual feedback, the mean power frequency of the intrinsic tongue muscle decreased over time, suggesting fatigue of the intrinsic tongue muscle. On the other hand, the suprahyoid muscle demonstrated compensatory increase of muscle activity, which contributed to maintaining tongue pressure
In a repeated tongue pressure task, the amplitude changes of motor evoked potentials of the tongue and suprahyoid muscles were observed when weak tongue pressure was repeatedly performed rather than strong tongue pressure. This was considered to be because application of weak tongue pressure is a dexterous movement accompanied not only by visual but also by somatosensory feedback in which the tongue presses against the probe.

Academic Significance and Societal Importance of the Research Achievements

摂食嚥下リハビリテーションの一つとして頻用される舌運動において,感覚入力がどのように影響しているかを検討した.視覚的フィードバック下で持続した強い舌圧発揮時には,舌を成す舌筋は急速に疲労するが,舌骨上筋が代償的に筋活動を増加させており,これは舌骨上筋の筋力を向上させるための訓練機序の解釈になると考えられた.また,繰り返しの舌圧発揮トレーニングと大脳皮質活動性の関係を検討したところ,力の制御が要求される弱い舌圧発揮の方が活動性変化が大きい結果となった.大脳皮質活動性変化を効果的に誘導するには,感覚入力による巧緻動作が求められる弱い舌圧発揮が有効であると考えられた.
