| Outline of Research at the Start |
１）認知症の保健・医療・介護連携体制に関わる専門職の、自認する役割と他職種・他機関から期待される役割の不一致（「役割の不一致」）を明らかにする。
２）１）で明らかになった「役割の不一致」の解消を目的とした、多職種連携教育プログラムを開発・実施する。
３）専門職を対象とした多職種連携教育が、実際の多職種連携に与える影響を検証する。

| Outline of Final Research Achievements |
Dementia support docters were expected to: (1) diagnose Alzheimer'sdisease, and (2) provide supervision. Additionally, primary care physicians expected them to: (3) initiate pharmacological treatment using anti-dementia drugs. On the other hand, it was found that unclear role definitions of dementia support doctors acted as a barrier to selecting them as a consultation resource. A common issue among healthcare professionals was that, although they recognized home-visit assessments as part of their role, there were barriers to actually carrying them out.
A training program incorporating the research findings was implemented. In addition, the results of this research were reflected in the overview of the Tokyo Metropolitan project "Tokyo Orange Doctor," which aims to promote interprofessional collaboration in the community by dementia support doctors.

| Academic Significance and Societal Importance of the Research Achievements |
認知症サポート医に期待される役割については、和文原著論文にまとめた。本稿によって認知症サポート医の現状と課題が明確になったとの評価を得て、老年医学雑誌に掲載された。
東京都地域連携型認知症サポート医（とうきょうオレンジドクター）の創設は、本課題によって明らかになった認知症サポート医の地域連携の障壁に対する解決策を探る一歩となる。
また、本課題で開発した教育プログラムは、とうきょうオレンジドクターを対象とした研修でも実施していく。
