Outline of Research at the Start

本研究は、ICU入室した患者とその家族を対象に、ICU退室3ヶ月後、6ヶ月後、12ヶ月後に身体機能、認知機能、メンタルヘルス、QOLに関するアンケート調査を行い、PICSの3要素の発症率とQOLとの関連性、PICS-Fの発症率とPICSとPICS-Fとの関連性を調査する。患者情報を日本集中治療医学会データベースであるJapanese Intensive care Patient Database(JIPAD)と包括医療費支払い制度(Diagnosis Procedure Combination: DPC)から抽出し、PICSデータベースと連結することで、post-ICUデータベースを構築する。

Outline of Final Research Achievements

In order to investigate the long-term prognosis of critically ill patients, a database of patients admitted to intensive care units (ICUs) was established. By the end of FY2024, 19 facilities had participated in the project, with 2,532 patients registered and a total of 1,410 questionnaires completed. The incidence of Post-Intensive Care Syndrome (PICS) three months after discharge from the ICU was 62.5%, of which 24.8% had physical dysfunction, 48.9% had cognitive dysfunction, and 33.8% had mental disorders. Furthermore, the incidence of PICS was 60.3% at six months after discharge from the ICU and 56.1% at twelve months, showing a similar trend in the proportion of each disability.

Academic Significance and Societal Importance of the Research Achievements

集中治療後症候群(post-intensive care syndrome: PICS)を対象とした継続的な調査データベースは、海外においても極めて稀である。PICSは文化的背景により影響を受けることが知られており、本邦における独自の疫学研究の報告は極めて重要である。本研究により、国内のICU入室患者の半数以上が長期的に後遺症を呈する実態が明らかとなった。得られた知見は、今後の医療政策や患者支援策の基礎資料となることが期待される。
