わが国は超高齢社会となり認知症を伴う高齢がん患者が増えていくことが自明です。中等度から重度の認知症がある患者は、痛みがあっても記憶障害のため痛みを忘れてしまったり、痛みをうまく伝えられず，大声をあげたり落ち着かない行動をとるなどで表現します。しかし医療者の知識不足により、これらを痛みの表現であると捉えられず見逃していることが多くあります。そこで我々は看護師に対して臨床判断を導くまでのプロセス（知識・思考過程・感情）の変容を促す効果があると言われているシミュレーション学習を取り入れた教育プログラムを開発し、認知症高齢がん患者の疼痛マネジメントの実践力を高め質の向上を図ることを目指します。

2023年度は、これまでに実施した前後比較調査で、知識テストが向上しなかった項目や実践自己評価が低かった項目に焦点を充てシミュレーション教育プログラムを洗練させていく計画で取り組んだ。知識テストが向上しなかった点については、スライド内容とデブリーフィングシナリオを見直した。実践自己評価が向上しなかった項目については、認知症高齢がん患者の臨床実践頻度の影響を受けている可能性もあるため、教育プログラムの内容は変更を加えないこととした。
