Outline of Research at the Start

年齢60歳以上の地域住民を対象として、ウェアラブルセンサによる歩行解析システムH-Gaitを用いたinstrumented timed up and go test(iTUG)を行ない、TUG課題中の股関節、膝関節および足関節の下肢キネマティクスおよび「ロコモ度」を評価する。さらに12か月後に第2次調査を行い、同対象に対して「ロコモ度」を再評価する。目的変数として第1次調査から第2次調査におけるロコモ度の重症化を、iTUGでのキネマティクスパラメータに加えて年齢、性別、BMI、体組成パラメータを説明変数として、多重ロジスティック回帰分析にてロコモ進行の危険因子を抽出する。

Outline of Final Research Achievements

The research project involved validating the application of the wearable sensor-based H-Gait system to TUG through comparisons with optical motion analysis and investigating kinematics factors to distinguish locomotive syndrome (LS). The validation revealed that correlation coefficients of 0.61 to 0.94 for the standing phase, 0.71 to 0.92 for the walking outbound phase, 0.76 to 0.88 for the walking return phase, and 0.72 to 0.80 for the sitting phase were observed, indicating the potential for application of H-Gait system to TUG. Furthermore, among 140 community-dwelling individuals aged 60 and older, hip extension angular velocity at the standing phase and knee joint range of motion at the walking return phase were identified as significant factors related to LS.

Academic Significance and Societal Importance of the Research Achievements

高齢化人口が急速に増加する中、ロコモティブシンドローム(ロコモ)の早期発見を目的とした動作解析システムの開発は喫緊の課題と考えられる。本研究課題により、短時間で測定・解析が可能なウェアラブルセンサーを使用したH-Gait がTUGテスト中の大規模な動作解析に応用可能であることを明らかにした。また、TUGテスト中の下肢キネマティクス評価は10m歩行テスト中の評価と比較し、ロコモを検出する上で非常に感度が高いことが明らかとなった。したがって、H-Gaitを用いたTUGテストはロコモを早期発見に有用であり、ロコモの予防および治療に有益と考えられ、健康寿命の延伸に大きく貢献できると考えられた。
