Outline of Research at the Start

脳動脈瘤の発生・成長・破裂に血流による力学的ストレスが関連すると示唆されて以来、数値流体力学(CFD)解析を用いて動脈瘤の発生・成長・破裂を誘起する流体力学的因子を定義する試みが行われてきた。しかし一般的な医学的統計研究と比較して、これまでのCFD研究の規模は非常に小さく、統計学的に血流-疾患の関連を明らかにする上で十分なサンプル数を確保していない。そこで本研究では、数万例の血流CFDを行うための抜本的な解析技術の転換として、深層学習により脳断面画像から生体内の流れ場を瞬時に推定する手法を提案し、その検証を行う。

Outline of Annual Research Achievements

本年度は学習データとして用いるCFD結果について境界条件が流れ場に与える影響を調べるため,超音波ドップラー計測による患者固有入口流量と直径に基づく推定入口流量を比較した.その結果,推定流量では過剰に流量を見積もる可能性が示唆された一方,WSSの空間分布は変化がないことが示唆された.また脳動脈瘤周辺血管の形状解析を行い,脳動脈瘤の位置および有無に伴う血管分岐が有意に異なることを明らかにした.血流推定ネットワークは血管形状と流れ場の関係を学習するものであるため,学習データには脳動脈瘤を伴うもの,伴わないものの両者を含める必要があると示唆される.これらの結果は査読付き英語プロシーディングスとして発表した.血流推定ネットワークの精度に関して,ネットワークに流れの支配方程式を損失関数として導入することで,物理的な妥当性を担保することを試みた.これにより,これまでの手法と比較して精度が向上し,拍動流解析に対し1秒以下で流れ場が出力することを可能とした.本結果は論文して取りまとめ,Computers in Biology and Medicineに掲載された.さらに本年度は脳血管形態の統計解析を行い,データ拡張を行うための新しい技術を開発した.多変量正規分布から変量をサンプリングすることで,実際の被験者群の非線形的な特徴を捉えた仮想患者データの大量生成が可能となった.本結果より2件の国際学会発表(うち招待講演1件),2件の国内学会発表(うち招待講演1件)に繋がった.また,大量生成した形状データに対するCFDの高速化に向け,深層学習とCFDを連成した高速血流解析手法の開発を行った.これにより,繰り返し計算の初期残差が減少し,安定して解析を行うことが可能となった.本結果は国際ポスター発表1件,国内学会口頭発表1件に繋がった.

2: Research has progressed on the whole more than it was originally planned.

Reason

医療画像から血流場取得までの工程のうち,本年度は血流推定の精度改善に向けた改良を行った.課題の1つ目として考えらえていた出力結果の精度保証について,PINNsの導入により流れの支配方程式を学習することが可能となった.またPINNsはこれまでと比較して必要な学習データを低減させることにも繋がるため,多様な患者個人形状への適用がより促進される.課題の2つ目である学習データの不足については,データ拡張手法として統計モデリングを導入することで解消される.本年度は動脈瘤を持たない脳血管形状について多変量正規分布を導入し,脳血管の多様性を再現可能なランドマーク点数の探索を行った.しかしながら一方で,脳動脈瘤疾患部の形状解析より脳動脈瘤形状と血管角度の相関が明らかになったことから,多様な形状を持つ脳動脈瘤患者のモデリング技術の開発を行う必要性が示唆された.多様な形状のモデリングのため,東北メディカルメガバンクで収集された医療画像コホートのプリプロセッシングを行う必要があるが,現在は手動で行う作業が多く,いまだボトルネック解消には至っていない.したがって,今後プリプロセッシングの自動化を行う必要がある.

Strategy for Future Research Activity

医療用画像から血流推定ネットワークを用いるまでの課程を自動化する必要がある.しかし撮像機器により取得される脳形態画像の特徴は異なるため,各工程をモジュール化し,パイプラインとして構築する.医療用画像から血管領域を判別する領域分割モジュールは各種撮像機器別に開発を行う必要がある.したがって,脳形態画像としてCFDで用いられるDigital Subtraction Angiography(DSA), Computational Tomography Angiography(CTA), Magnetic Resonance Angiography(MRA)画像を用いて,領域分割深層学習ネットワークを構築する.同一のGrand Truthを用い各撮像機器の領域分割結果について精度評価を行う.血管領域に対して血流推定点を設定する点群生成モジュールは共通で用いることが可能である.出力する点群数を制御し,点群数の違いによる推定結果の差を評価する.これらのモジュールを組合せ,医療画像より血流場を推定可能なソフトウェアとして構築する.
