Outline of Research at the Start

多施設共同研究において蒐集した成人T細胞白血病/リンパ腫の大規模症例コホートを対象として、CCR4などの遺伝子異常の臨床的意義を確立するとともに、最近の申請者らの予備的検討により重要な意義を有することが示唆されたCCR7遺伝子変異について詳細に解析を進める。また、一部のリンパ系腫瘍や種々の固形癌において治療標的となっているNOTCH1やCDKN2A遺伝子異常が本腫瘍において高頻度に認められることを見出している。成人T細胞白血病/リンパ腫におけるこれらの遺伝子異常の臨床的意義を解明し、治療標的分子としての可能性を検証する。その上で検索範囲を末梢性T細胞リンパ腫全体に広げて、本研究を発展させる。

Outline of Final Research Achievements

Adult T-cell leukemia/lymphoma (ATL) is relatively rare tumor, and a large number of this tumor cases were collected through multicenter joint research and clinical pathological analysis was performed. We reported that gene alterations such as CCR4, CCR7, and NOTCH1 are significant prognostic factors in ATL. It is hoped that these studies will promote molecular pathology of ATL and that it will be directly and indirectly applied to the stratified and individualized treatment for patients with ATL.

Academic Significance and Societal Importance of the Research Achievements

本研究では、多数のATL症例を蒐集してゲノム解析を実施した。 臨床情報との統合解析により、ATLにおいて高頻度に認められる遺伝子異常の詳細な臨床的意義を明らかにした。 本研究により得られた成果は、これらの遺伝子変異の有無を基盤とするATLに対するプレシジョンメディシン確立を提案するものである。 今後は、ATLにおける各種因子・分子や遺伝子異常と現時点での最適なATLの治療選択を総合化し、一般の血液治療者にATL治療アルゴリズムを明確に提示することが重要な課題である。
