Outline of Research at the Start

本研究では腹部超音波検査、動脈硬化検査、血液検査所見などを併用し、心不全患者を対象とした観察研究と動物を対象とした血行動態把握の実験を行う。循環動態としてのうっ血の有無や低灌流の有無を非侵襲的に評価する手法を確立することを目的としている。これらにおり心不全患者に他の臓器障害の合併を認めた時に、うっ血解除(利尿薬の使用など)と低灌流改善(補液や強心薬の使用など)のどちらが臓器障害改善に有効かを予測することができるようになり、今後の薬物治療の副作用回避や治療期間の短縮に役立つ可能性がある。

Outline of Final Research Achievements

In accordance with the research plan, non-invasive hemodynamic evaluation using abdominal ultrasonography, blood tests, cardio-ankle ,vascular index, ClearSight, etc. was performed in patients with heart failure during a stable period before discharge. Registration of eligible patients has been completed. We analyzed the collected data and reported the high reliability of congestion evaluated by abdominal ultrasonography and the relationship between various non-invasive evaluation variables and prognosis at related conferences, and some of the results were accepted as English papers.

Academic Significance and Societal Importance of the Research Achievements

本研究により、過去の侵襲的な血行動態検査に頼らずとも非侵襲的に心不全患者の血行動態が把握できることが示唆された。これにより今後の患者管理がより安全となり、また必要時に用意に再評価できることが期待される。
