Outline of Research at the Start

近年、てんかん発作時の広帯域皮質脳波(低周波活動と高周波活動)が、てんかん焦点を示唆する所見として注目されている。しかし、これまで発作時の広帯域皮質脳波は目視により評価されており、評価者による解釈の違いや解析に時間がかかる点が課題であった。本研究では、この課題を解決するべく、これまでの申請者の成果を発展させ、てんかん外科の術前に皮質脳波を記録した患者で、発作時の広帯域皮質脳波の定量化を試みる。さらに、定量化した指標が他の臨床検査所見や術後の発作予後と相関するかを検証し、特異度と汎用性の高いてんかん焦点のバイオマーカーとしての可能性を探索する。

Outline of Annual Research Achievements

研究項目1) 発作時の広帯域皮質脳波の定量化
前年度に引き続き、当院で頭蓋内電極を留置し、てんかん外科手術を施行した難治てんかん患者を後方視的に解析対象とした。① てんかん発作時の低周波活動の定量化:てんかん発作時の低周波活動について、持続時間、振幅・パワーを、最も早期あるいは大きな変化を示すコア電極に関して解析した。起始点・ピーク・終止点を自動で同定するプログラムを作成した。② てんかん発作時の高周波活動の定量化:てんかん発作時の高周波活動について、持続時間(起始、停止)、周波数帯域とその変動、パワーの解析をまずはコア電極を中心に開始・継続した。③ 低周波・高周波活動の半自動的同定手法の精度の検証:発作時の低周波活動の半自動的な抽出法の精度を、深層学習の手法を用いて検証した。テストデータでの発作・発作時の低周波活動の起始部の検出精度はそれぞれ0.88と0.80であった。深層学習による発作時の低周波活動の検出を行った(国内学会発表、国際学術論文へ投稿準備中)。この技術の精度向上・実用化を進めている。
研究項目2) 定量的なてんかん発作時低周波・高周波活動と臨床所見やてんかん外科治療の術後予後との相関の検証
他施設との共同研究で、時定数2秒で記録されたてんかん発作時頭蓋内脳波の低周波・高周波活動をコア電極で評価した。てんかん発作時低周波・高周波活動とも、コア電極部位の切除が、術後の良好な発作予後と有意に相関することを明らかにし、関連国際学術論文へ投稿、採択された(Izumi M, Kobayashi K, et al. Epilepsia. 2023;64:3294-306.)。てんかん発作時低周波・高周波活動と術後予後との相関を明らかにするとともに、従来は時定数10秒で記録・評価されていた発作時低周波活動を時定数2秒で記録・解析することで、その意義も明らかにすることができた。

研究項目1) については、てんかん発作時の広帯域皮質脳波のうち、低周波活動に関して深層学習による半自動的同定も行えている。
研究項目2) については、視察的評価による解析であるが、てんかん発作時低周波・高周波活動と術後予後との相関を明らかにできた。

Strategy for Future Research Activity

今後は、てんかん発作時の広帯域皮質脳波のうち、高周波活動に関しても解析を進める。
