Outline of Research at the Start

周産期医療の発達により新生児外科疾患の救命率は飛躍的に向上し、近年は神経学的予後が重要なアウトカムとして位置づけられる。腸内フローラ異常(dysbiosis)の是正が新生児外科疾患の急性期治療に有効であるとの知見は散見されるが、その長期的効果、特に中枢神経発達への影響は未知である。本研究では新生児壊死性腸炎モデルラットを用い、腸内フローラを治療ターゲットとして急性期治療効果を検討するだけでなく、拡散テンソルMRI画像を用いて中枢神経の発達を評価することで、長期的な神経学的予後を見据えた腸内フローラ制御・管理方法の開発を目的とする。

Outline of Final Research Achievements

This study aimed to develop a new therapeutic strategy targeting the gut microbiota to improve long-term neurological outcomes in neonatal necrotizing enterocolitis (NEC). We attempted to create NEC model rats, but consistent model generation was challenging. As an alternative, we used a model combining massive small bowel resection and total parenteral nutrition to evaluate the effects of clodronate and omega-3 fatty acids on the intestine and liver. Omega-3 fatty acids showed a protective effect against intestinal mucosal atrophy and hepatic steatosis, while clodronate was effective in preventing hepatic fat accumulation, though its intestinal effect was limited. Although histological analysis of the brain using HE staining was conducted, it was insufficient to assess central nervous system impact. Due to delays in the experimental process, MRI evaluation could not be completed, and further studies are required to fully explore the neurological outcomes.

本研究テーマは新生児外科疾患患児の神経学的予後を腸内フローラの制御・管理により改善するという新たな視点からの研究である。NECをはじめとする新生児外科疾患は症例数が限られており、動物モデルによる検討は極めて重要であるが、NECモデル作成の実験手技は煩雑で、モデル確立に技術を要する。腸内フローラの異常が神経学的発達に与える影響を解明することができる。
