Outline of Research at the Start

本邦では、高齢化に伴い虚血性心疾患が増加、患者背景も多様化しつつあり、患者個人に応じた最適な治療を提供することが求められている。しかし、患者背景の多様化により、ランダム化比較試験の結果を、最適な治療として適応できない状況も増えている。本課題では、機械学習、統計学的評価を用いることで、重症冠動脈病変を有する患者に対する最適な冠動脈血行再建術、経皮的冠動脈形成術後の最適な抗血小板療法を、患者背景に応じて提供する治療選択決定モデルの作成を目指す。

Outline of Annual Research Achievements

高齢化に伴い虚血性心疾患が増加、患者背景も多様化しつつあり、患者個人に応じた最適な治療を提供することが求められている。一方で、ランダム化比較試験の結果が最適な治療として適応できない状況も増えている。
ランダム化比較試験のデータベースに機械学習、統計学的評価を適応させることで、精度の高い治療選択決定モデルを作成することを目指している。また、その結果を実臨床で用いることで、より良い治療を提供するとともに、そのモデルの妥当性も検証する。
機械学習を用いることで、治療選択の際に検討すべき要因として、炎症反応マーカーや健康関連のQOL評価があること、それらを治療選択決定モデルに導入することでよりよりモデルを構築できることを見出した。モデルを作成したデータベース以外のデータを用いての最適化を行っている。また、実臨床での有効性を検討するための準備も行っている。
経皮的冠動脈形成術後の最適な抗血小板療法（種類・期間）のモデルに関しては、抗血小板薬に関する新たなエビデンス（経皮的冠動脈形成術直後のアスピリンの有無に関して）が提示されていることもあり、データのクリーニングに時間を要している。冠動脈血行再建術の治療選択モデルで機械学習のアルゴリズムの最適化にも取り組んでいるため、そのアルゴリズムを適応させることで、効率よく治療選択モデルの作成を行っていく。
