Outline of Research at the Start

申請者は悪性胸膜中皮腫(MPM)の前臨床モデルを用いた腫瘍免疫研究に従事してきた中で、マウスMPM同所性腫瘍モデルの腫瘍内微小環境(胸水)中のマクロファージに、腹腔内マクロファージと同様のsubsetがあることを発見した。MPM進展に伴って、そのsubsetの1つであるsmall pleural macrophages(SPM)が腫瘍免疫抑制性に働くM2 phenotypeを獲得していることを見出し、SPMが中皮腫の進行に関与している可能性が示唆された。本研究の目的は、マウスMPM腫瘍内微小環境におけるSPMの意義を明らかにし、SPMを標的とした新規免疫療法・集学的治療の開発を行うことである。

Outline of Final Research Achievements

Tumor immune microenvironment (pleural effusion) of a murine orthotopic mesothelioma model was analyzed, and two distinct macrophage populations, monocyte-derived small pleural macrophages (SPMs) and tissue-resident large pleural macrophages (LPMs), were identified. SPMs were tumorigenic. Deletion of Trem2 gene, which we identified from gene signature analysis of SPMs, led to reduced SPM M2-like tumor-associated macrophages with compensatory increase in LPM and slower tumor growth. SPMs may be a potential therapeutic target for novel immunotherapy for malignant pleural mesothelioma.

Academic Significance and Societal Importance of the Research Achievements

免疫チェックポイント阻害剤など各種治療の進歩にも関わらず、悪性胸膜中皮腫(MPM)の予後は依然として極めて不良であり、新たな治療法の開発が急務である。腫瘍関連マクロファージ(TAM)は、様々な癌種において悪性度に関与しており、これを標的とした治療の有効性がマウスなどの前臨床モデルを用いた研究で報告されているが、一般臨床への応用には未だ至っていない。本研究により、MPMの腫瘍内免疫微小環境に2つのマクロファージサブセットがあることが同定され、腫瘍免疫抑制性に働く、単球由来のsmall pleural macrophagesがMPMに対する新規免疫療法の標的となりうることが示唆された。
