Outline of Research at the Start

既存の固定法は組織の硬化や軟化,髄膜の脆弱性を来すため脳血管内治療のCSTに適さない.さらに,御遺体の血管を視認するための造影剤の組成及び灌流方法は報告がない.我々が開発したTEIFは脳及び髄膜の性状を維持する新たな固定法であり,脳脊髄液を置換する固定液によって頭蓋内血管を含む硬膜下組織の硬度や柔軟性を自由に設定できる潜在性がある.
このような点から,TEIFを改良した新規脳組織固定法の開発が脳血管内治療のCSTの確立に寄与する可能性を着想した.本研究は「脳血管の変性を回避し,凝血のない固定法とは何か」を問いとする.

Outline of Annual Research Achievements

脳血管内治療のためのCadaver Surgical Trainingを確立するにあたり、脳血管内治療の現状の課題を評価するため、まずは脳血管内治療の臨床的側面に焦点を当て、特に内頚動脈及び総頚動脈病変に対する血管内治療についての検討を行い、査読付き英文誌に掲載した。(Shigeta Miyake: Efficacy and safety of thrombectomy for acute ischaemic stroke in patients with pre-stroke mRS scores of 2-3: Real-world evaluation from an open-label, prospective, multicentre, observational study.

さらに、御献体を用い、側頭骨を除去することで内頚動脈を損傷することなく全体を露出・摘出することを可能とし、脳血管内治療のためのCSTのアクセスルートの評価を可能とした。

Reason

脳血管内治療のためのCadaver Surgical Trainingを開発するための動物実験を行うにあたり、研究課題の焦点を絞るための臨床的評価及び御献体を用いた解剖研究を先行した。
今後動物実験を計画、実施する方針である。

Strategy for Future Research Activity

動物を用いた固定法の研究に進み、脳血管内治療のSurgical Trainingを確立するための準備を進める。
