本研究では、in vitroでの角膜内皮細胞や線維柱帯細胞へのCMV感染実験で、性ホルモンや加齢性変化、それに伴う酸化ストレスなどが、角膜内皮細胞の細胞死や線維柱帯細胞の線維性変化に与える影響を検討し、その仮説を検証する。また、患者の前房水や血液サンプルを蓄積し、これらの検証を行う。

| Outline of Final Research Achievements |
Cytomegalovirus anterior segment infection is characterized by recurrent inflammatory attacks accompanied by elevated intraocular pressure, also known as secondary glaucoma, decreased corneal endothelial cell density, and corneal edema. Secondary glaucoma often proves to be refractory, frequently necessitating glaucoma surgery to reduce intraocular pressure. The results of this study suggest that in cases of cytomegalovirus anterior segment infection, the invasiveness of glaucoma surgery may influence the decrease in corneal endothelial cell density through the induction of cytomegalovirus activation and alterations in the anterior chamber environment.

| Academic Significance and Societal Importance of the Research Achievements |
サイトメガロウイルス前眼部感染症は疾患概念の確立からまだ比較的歴史が浅く、その病態については未知な部分が多い。その一方、前房水PCRの普及により、確定診断例は急増しているという背景がある。疫学的に中高年男性に多いという特徴があり、性差や加齢がその病態に関連している可能性があり、そういったアプローチによる今後の病態解明が期待される。また、本研究で示唆された、緑内障手術がその病態に与える影響についても、今後分子生物学的、免疫学的なアプローチによる解明により、より適切な術式選択や術後管理といった臨床的な課題の解決に資する可能性がある。
