Outline of Research at the Start

超高齢社会を迎えた本邦において,摂食嚥下障害の病態解明は,医療介護分野で求められている安全な食支援につながる重要な課題であるが,咀嚼過程の中で,口腔感覚入力が及ぼす影響を食塊形成や続く嚥下運動に与える影響という観点で調べた研究は少ない.本研究では,口腔内への麻酔により口腔感覚障害を模した健常者を対象に,嚥下造影検査ならびに筋電図記録の解析を行うことで,口腔感覚入力が,咀嚼と嚥下に及ぼす変調効果について明らかにする.

Outline of Final Research Achievements

This study aimed to clarify the effects of oral sensory impairment on mastication, bolus formation and transport, and swallowing. Using local anesthesia applied to one side of the dentition, we simulated unilateral oral sensory loss in healthy adults and conducted surface electromyography and videofluoroscopic examination of swallowing. The results revealed that sensory loss led to decreased masticatory efficiency, disrupted chewing rhythm, reduced muscle activity, and prolonged bolus transport time. These findings suggest that oral sensory input plays a critical role in the coordination and timing of feeding and swallowing functions. Further investigations involving older adults and patients with dysphagia are needed to evaluate how oral sensory deficits affect swallowing safety and efficiency, with the ultimate goal of developing more effective assessment and rehabilitation strategies in clinical practice.

Academic Significance and Societal Importance of the Research Achievements

本研究は、口腔感覚障害が食事に関わる機能、特に咀嚼、食塊形成・移送、嚥下に与える影響を解明した。健常成人を対象に実施した実験により、口腔感覚の喪失が咀嚼リズムの乱れや筋活動の減少、摂取時間の延長を引き起こすことが明らかになった。この結果は、摂食嚥下障害を抱える患者の治療において、口腔感覚機能の重要性を再認識させるものであり、今後、口腔感覚障害を持つ患者に対するより効果的なリハビリテーション方法や支援策の開発が期待される。また、臨床現場における適切な支援方法の検討に寄与することが期待される。
