Outline of Research at the Start

ランダム化臨床試験は、治療の有効性・安全性を評価するうえでゴールドスタンダードの研究デザインであるが、割付治療開始以降の事象（例：治療変更による時間依存性交絡、死亡による欠測、追跡不能による打ち切り）は、一般的にバイアスを引き起こす。バイアスのない治療効果を推定するための手法として、統計的因果推論の手法が提案されている。本研究では、比較的小規模なデータからもバイアスが小さく解釈可能な治療効果を推定するための方法を開発し、その性能評価を行うことを目的とする。

Outline of Annual Research Achievements

ランダム化臨床試験は、治療の有効性・安全性を評価するうえでゴールドスタンダードの研究デザインである。ICH E9 R1でも指摘されているように、患者の追跡開始後に中間イベントが起こりうる場合、推定したい治療効果（estimand）を設定することが肝要である。本年度は、昨年度に引き続き、興味のあるイベントに対する競合イベントが存在する状況におけるestimandを検討した。割付治療が興味のあるイベントに与える様々な直接効果の推定上の仮定を整理し、それらの違いを明確にするシミュレーション実験のデザインを行った。

今年度は、過去年度の研究成果に基づき、シミュレーション実験のデザインを行えた。また、類似研究の調査を行い、実施中の研究の位置づけを確認できた。

今後はシミュレーション実験の成果のとりまとめ、および実データ解析へと進める。
