| Outline of Research at the Start |
わが国では看護管理者の資質向上のため、大学院教育や認定看護管理者教育が行われ、専門的教育を受けた看護管理者の養成が進められている。しかし看護サービス管理の専門家である専門的教育を受けた看護管理者の配置と患者アウトカムとの関連は検証されていない。
本研究では、専門的教育を受けた看護管理者の配置と患者アウトカムとの関連についてリアルワールドデータを用いて明らかにする。本研究により、患者アウトカムに貢献する専門的教育を受けた看護管理者の配置の重要性を示すことができるとともに、どのような教育が看護管理者の実践を変え、その配置が患者アウトカムの向上に貢献したのかを解明する手掛かりが得られる。

| Outline of Final Research Achievements |
This study aimed to clarify the relationship between the placement of nurse managers with specialized education and patient outcomes through three separate investigations. First, the geographical and institutional distribution of Certified Nurse Administrators (CNAs) was described. The results showed that CNAs were predominantly concentrated in urban areas, near educational institutions, and in large public hospitals. Second, the relationship between patient outcomes and departments managed by nurse managers who had completed CNA training or graduate school was examined. Although the differences were not statistically significant, a trend toward better patient outcomes was observed in departments managed by educated nurse managers. Finally, the association between CNA affiliation at facilities and the home discharge rate of departments was analyzed. The results indicated that facilities with CNA affiliation had significantly higher home discharge rates compared to those without.

| Academic Significance and Societal Importance of the Research Achievements |
これまで認定看護管理者をはじめとする教育を受けた看護管理者が組織や患者に与える効果についての研究は、認定看護管理者自身の認識を質的に分析したものにとどまっていた。しかし本研究により、他の高度実践家（認定看護師・専門看護師等）と同様、看護管理の専門家としての看護管理者が、組織や患者アウトカムに良い効果を与え得ることが明らかにされた。一方、専門的教育を受けた看護管理者は、地域や施設に偏在していることも確認され、場所や施設属性によって看護管理者の専門的教育効果を受けられるかどうかが左右されることもまた実証された。本研究は、今後の看護管理者への専門的教育の詳細な効果検証に寄与する基礎研究である。
