本研究は、心不全患者、急性期病院、訪問看護を中心としたICTツールを活用したシームレスな心不全管理システムを構築することを目的とする。
既存の遠隔心不全管理ツールは、患者の日々のバイタルサインや自覚症状をグラフ化し、セルフケア行動の維持、促進および訪問看護師のアセスメントの焦点化や症状増悪への早期発見早期対処を行い、再入院予防やQOL向上を目指すものであった。本研究では、病院や在宅を問わず活用可能なツールへ拡充し、平常時の看護業務の効率化、病院と地域間の移行の円滑化によるシームレスな心不全管理、緊急時における迅速で簡潔な情報共有を可能とする看護師間の連携システムを構築し、実装に向けた評価を行う。

| Outline of Final Research Achievements |
Toward seamless heart failure management using ICT and strengthening regional collaboration, we conducted a needs survey regarding the transition period for medical professionals in hospitals and home setting. The results revealed that the information needed during the transition period for heart failure differed in part depending on the type of job and characteristics of the affiliation.
In addition, to improve the quality of heart failure management and serve as a bridge for regional collaboration, we developed a heart failure exacerbation response flow and heart failure symptom scoring system that can be used regardless of setting, such as in hospitals or home setting, and that allows for the sharing of patient information in real time, and examined the contents using a Delphi survey. Based on the results, we obtained suggestions for establishing a highly feasible heart failure management system that utilizes ICT.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、各医療専門職ごとに発展してきた心不全管理の効果的な方法について、多職種協働の視点で、ニーズ、介入方法を検討した数少ない研究である。特に、多職種多施設を対象としたニーズ調査では、これまで単一の職種のみの研究成果がほとんどである中、包括的な観点から心不全の移行期ケアの新たな方略の開発に寄与する知見の創出があった。
また、心不全症状早期発見に向けた対応フローおよびスコアリングシステムにおいても、多職種多施設の医療専門職を対象としてデルファイ調査を実施したことで、実臨床における実現可能性の高い調査結果を得ることができた。
