Outline of Research at the Start

脊髄損傷患者の再生医療においては、損傷部の瘢痕化や、麻痺した手足の筋・関節の廃用性機能低下、随意運動に必要な神経回路を再編する必要がある点などから、移植治療と薬剤管理・リハの併用が望ましいと考えられている。申請者は以前の研究で、隔離飼育により作製した社会性低下モデルラット群でグループ飼育群より接近・接触行動が減少し、その神経基盤と考えられる内側前頭前野の神経活動変化を認めた。この結果より、動物においてもリハ手法が効果に影響すると考えた。そこで、脊髄損傷モデルラットのリハとして自発的運動が有効ではないか、また自発的運動の併用により脊髄再生治療の効果が増強されるのではないかという点を明らかにする。

Outline of Final Research Achievements

We performed rehabilitation once a day, five days a week, on spinal cord injury model rats, and confirmed that walking function improved significantly from the sixth week onwards in both the voluntary and forced exercise groups compared to the control group, and that regenerated axons extended longer than in the control group. It was also confirmed that when forced exercise was performed twice a day, five times a week, with increasing duration, walking function improved significantly from the fifth week onwards compared to the control group. These findings suggest that voluntary exercise with increased load may improve walking function in the longer term, so we will continue this investigation.

Academic Significance and Societal Importance of the Research Achievements

再生医療の研究動向として、①細胞移植と、②神経栄養因子や再生阻害因子抑制剤を投与し再生環境を整える、③損傷部の細胞欠損部を埋める足場剤を補充する、④リハビリ介入により脊髄機能の自己修復能を高める、等を併用する方法が検討されている。本研究は上記④に該当し、①~③では回復に限界があるとされている現状において、リハビリ併用に対する期待が大きい。我々が手がけてきた体性幹細胞を用いた脊髄再生医療は、安全性において臨床応用が進めやすいという利点がある。その効果をより高めるリハビリ手法を開発することは社会的に意義が大きい。
